髓内钉法治疗股骨转子周围骨折的临床效果观察(1)

【摘要】目的 探讨髓内钉法治疗股骨转子周围骨折的临床疗效,总结其临床意义。方法 选取我院2007年6月至2009年6月到骨科就诊的51例股骨转子周围骨折患者,男性31例,女性20例,年龄67~90岁。其中稳定型骨折(A1.1~A1.3)23例,不稳定型骨折(A2.1~A3.3)28例。对51例患者应用FixionPF型髓内钉法进行治疗,记录相关数据,进行统计分析。结果 51例患者的伤口全部在Ⅰ期愈合。手术时间为(75±25)min,平均65min。术中出血量(160±90)mL,平均136mL。随访8~26周,平均14周,骨折平均愈合时间10周。术后髋关节功能恢复优18例,占35.5%;良23例,占45.1%;中8例,占15.7%;差2例,占3.9%;总有效率80.4%,手术治疗后髋关节功能效果显著(P<0.05),具有统计学意义。结论 髓内钉法治疗老年性股骨转子周围骨折具有手术操作简便、创伤小、手术时间短、大大减少及X线下暴露时间等优点,有效减少医生和患者的放射损害,应用髓内钉法压松质骨,降低了内固定物对骨质的切割,骨折愈合加快。髓内钉法治疗股骨转子值得临床合理推广使用。

【Abstract】Objective To investigate the treatment of intramedullary nailing of intertrochanteric fractures of the clinical efficacy, summarize its clinical significance. Methods Our hospital from June 2007 to June 2009 to the orthopedic treatment of 51 cases of patients with intertrochanteric fractures, 31 males and 20 females, aged 67 to 90 years old. One stable fracture (A1.1~A1.3) 23 cases of unstable fractures (A2.1 ~ A3.3) 28 cases. 51 cases of patients FixionPF type intramedullary nail method for treatment, record the data for statistical analysis. Results All 51 patients in stage Ⅰ wound healing. Operative time (75±25) min, an average of 65min. Blood loss (160±90) mL, an average of 136mL. Excellent functional recovery after hip in 18 cases, 35.5%; good in 23 cases, 45.1%; in 8 cases, 15.7%; poor in 2 cases, accounting for 3.9%; total efficiency 80.4%, surgical treatment of hip joint function significant effect (P <0.05), with statistical significance. Conclusion Treatment of senile nail intertrochanteric fractures with a simple surgical operation, trauma, surgery time is short, and greatly reduce the X-ray exposure time, etc., doctors and patients to effectively reduce the radiation damage, the application of intramedullary nailing pressure cancellous bone, reducing the internal fixation of bone cutting, accelerating fracture healing ......
