【摘要】目的 评价三阶梯技术对宫颈上皮内瘤变(CIN)筛查依次进行的临床应用价值。方法 随机选择2009年1月～2010年9月南昌市妇幼保健所妇科普查及妇科门诊宫颈液基细胞学涂片检查异常者120例作为实验组,同时选择120例未行宫颈液基细胞学涂片检查者作为对照组, 按宫颈糜烂程度、临床症状及年龄、孕产次与实验组进行1：1配对。两组分别行阴道镜检查宫颈定点活检送病理，以组织学诊断为金标准，对宫颈上皮内瘤变进行筛查，比较两组患者CIN的检出率。结果 实验组及对照组的CIN的检出率分别为72.5%、16.7%,x2= 75.71，P<0.01，其结果差异有显著性。结论依次按三阶梯技术进行宫颈病变筛查可提高宫颈上皮内瘤变(CIN)和宫颈癌的检出率，是筛查和诊断宫颈病变的可靠手段。

【Abstract】Objective To evaluate the three-step technique of cervical intraepithelial neoplasia (CIN) in order to conduct clinical screening application.Methods A randomly selected in January 2009 ~ September 2010 Nanchang MCH outpatient gynecology and gynecological screening cervical cytology smear abnormalities in 120 patients as experimental group, while 120 cases of cervical cytology is not painted Film examination as control group, according to cervical erosion, clinical symptoms and age, maternal time and experimental group 1:1. Two groups were performed colposcopy cervical biopsy sent to pathology point to histological diagnosis as the gold standard of cervical intraepithelial neoplasia screening, two groups were compared the detection rate of CIN. Results The experimental and control groups, respectively, the detection rate of CIN was 72.5%, 16.7%, x2 = 75.71, P <0.01, the results were significantly different. Conclusion The technique followed by a three-tiered screening of cervical lesions can improve cervical intraepithelial neoplasia (CIN) and cervical cancer detection rate, screening and diagnosis of cervical lesions is a reliable means.

目前，大量研究表明，位居女性恶性肿瘤第二位的宫颈癌是唯一一种可经过医学干预能使其发病率和死亡率下降的恶性肿瘤[1],预防和控制宫颈癌的关键是早期诊断和治疗癌前病变——宫颈上皮内瘤变(CIN)，阻断其进展到宫颈癌的潜在危险。因此合理而有效的筛查方案能降低宫颈癌的发病率。宫颈疾病筛查三阶梯技术是：宫颈液基细胞学检查、阴道镜检查、组织学检查。南昌市妇幼保健所按照三阶梯技术依次进行宫颈上皮内瘤变筛查,评价三阶梯技术在宫颈病变筛查中的临床应用价值 ......
