【摘要】目的 观察布比卡因联合芬太尼对剖宫产术后产妇的镇痛效果。方法 200例ASA Ⅰ—Ⅱ级的剖宫产的产妇平均分两组。A组术后给予硬膜外镇痛，B组给予传统肌注镇痛药。镇痛液配方：0.75%布比卡因20ml、芬太尼0.3mg、氟哌啶5mg，加生理盐水至100ml，设置负荷剂量为0.375%布比卡因10ml，背景剂量为2ml/h，记录术后36h产妇的心率、血压、呼吸频率、脉搏、血氧饱和度，VAS镇痛评分以及不良反应的发生率。结果 A组100例应用术后镇痛的产妇均取得良好的镇痛效果，与B组有显著差异。结论 长效局麻药布比卡因联合阿片类受体激动剂芬太尼用于剖宫产术后镇痛，能够获得良好的镇痛效果，不良反应发生率极低，优于传统肌注镇痛药的镇痛方式。

【Abstract】Objective To observe the bupivacaine combined with fentanyl for analgesia after cesarean section maternal effect. Methods 200 cases of ASA Ⅰ - Ⅱ grade average maternal cesarean section divided into two groups. A group of patients given epidural analgesia, B group received conventional intramuscular analgesics. Analgesic liquid formulations: 0.75% bupivacaine 20ml, fentanyl 0.3mg, droperidol 5mg, add saline to the 100ml, set the loading dose of 0.375% bupivacaine 10ml, background dose of 2ml / h, record 36h after maternal heart rate, blood pressure, respiratory rate, pulse, oxygen saturation, VAS pain score and the incidence of adverse reactions. Results A group of 100 cases of maternal analgesia were applied to achieve good analgesia, and group B were significantly different. Conclusion The long-acting local anesthetics bupivacaine combined opioid receptor agonist fentanyl for analgesia after cesarean section, can be a good analgesic effect, low incidence of adverse reactions, than conventional intramuscular injection analgesic analgesia.

本文对布比卡因联合芬太尼用于剖宫产术后硬膜外镇痛的效果进行观察。
1 资料与方法
1.1 一般资料 200例年龄20-32岁、ASAⅠ—Ⅱ级剖宫产的产妇随机分两组。A组100例，给予术后硬膜外镇痛。B组100例传统肌注镇痛，术后镇痛液：0.75%布比卡因20ml、芬太尼0.3mg、氟哌啶5mg，加生理盐水至100ml。排除标准：对研究的药物过敏；不能使用PCA；无法交流；有肝、心、肺、肾疾病；长期应用麻醉性镇痛药者等。全部剖宫产手术均采用腰2、3， 腰麻-硬膜外联合阻滞、见脑脊液流出后，腰麻针孔向天，推注2%利多卡因1.3-1.6ml，推注时间为12-15秒。术毕前硬膜外推注镇痛液0.375%布比卡因10ml后接术后镇痛泵。
