甘精胰岛素联合格列美脲片治疗2型糖尿病的临床疗效观察(1)

[摘要]目的评价甘精胰岛素联合格列美脲对于二甲双胍治疗血糖控制不佳的2型糖尿病患者疗效及安全性,并且在B细胞功能保护方面有无优势。 方法148例既往使用饮食控制、运动加二甲双胍治疗血糖控制不佳的2型糖尿病患者,随机分为两组,分别采用甘精胰岛素(来得时)+格列美脲片(亚莫利)(A组)或诺和灵30R(B组)治疗12周,以糖化血红蛋白、空腹血糖及餐后2h血糖评价其有效性;根据低血糖反应及血生化指标的变化,评价其安全性;比较两种治疗方法在安全性、疗效及B细胞功能保护方面有无差别。结果治疗12周后,两组患者的糖化血红蛋白、FBG、PBG均较治疗前显著下降,差别有显著意义,但组间比较差别无显著意义;低血糖反应诺和灵30R治疗组明显高于来得时+亚莫利组;胰岛B细胞功能指数(MBCI)治疗后均升高,其中来得时治疗组更显著,差别有显著意义。结论来得时联合亚莫利治疗,有效、平稳地控制24h血糖,低血糖反应少,实现糖化血红蛋白达标<6.5%,改善胰岛细胞功能。

An efficiency about Glargin add Amaryl in treating type 2 diabetes mellitus

[Abstract]ObjectiveTo evaluate the efficiency and safety of long-acting insulin analogue on type 2 diabetes patients with the level of blood glucose poorly controlled formerly,whether it has the protective effects on the function of islet or not. Methods 128 type 2 diabetes patients with the level of blood glucose poorly controlled formerly were enrolled and treated with Glargin+Amaryl(group A)or Nova insulin 30R(group B) for 12 weeks. The efficiency was evaluated through HbA1C,FBG and 2hPBG after 12 weeks of treatment. The safety was evaluated through the adverse events including hypoglycemic episodes and the changes of clinic laboratory parameters.Compare the two therapies on safety, efficiency and the protective effects on the function of islet. Results After 12 weeks of treatment,the HbA1C,FBG and 2hPBG were decreased dramatically.The differences were significant,but the differences between the two groups were not significant.The hypoglycemic episodes in group B were more than in group A.MBCI were increased after treatment,but group A was more significant. ConclusionThe treatment of Glargin add Amaryl can control blood glucose effectively and stably,lower hypoglycemic episodes,realize the objection of HbA1C<6.5% and improve the function of islet.
