[摘要] 目的 探讨24小时动态心电图对房性早搏诱发阵发性心房颤动的临床意义。方法 对48例房性早搏诱发的阵发性房颤患者的动态心电图分析，测量并计算诱发房颤的房早联律间期、房早前周期及房早联律间期/房早前周期比值，未诱发房颤的房早联律间期、房早前周期及房早联律间期/房早前周期比值。结果 48例阵发性房颤患者中均伴有频发房早、短阵房速，未见心房扑动。24h心电图平均房早 (10998±1896)个，阵发性房颤(232±46)阵，其中40例(83.3% ) 由房早诱发，8例(16.7%) 由短阵房速诱发。诱发房颤的房性早搏其房早联律间期与未诱发房颤的房早联律间期，房早前周期及房早联律间期/房早前周期比值差异有统计学意义(P<0.01)。结论 动态心电图作为一种无创伤检查能够确定阵发性房颤的始动因素，可以作为临床选择治疗方式及观察疗效的一个重要工具。

Clinical significance of evaluating paroxysmal atrialfibrillationinducedby atrial premature beats with dynamic electrocardiogram

[Abstract] Objective To investigate the clinical significance of evaluating paroxysmal atrial fibrillation (PAF) induced by atrial premature beats with 24 hours dynamic electrocardiogram(DCG). Methods48 patients with PAF induced by atrial premature beats were selected and their DCG results were analyzed. Cycle of ectopic beat of atrial premature beats and cycle before atrial premature beats of all patients were measured and the ratio of cycle of ectopic beat of atrial premature beats to cycle before atrial premature beats were calculated. Results48 patients with PAF all had frequent atrial premature beats and paroxysmal atrial tachycardia but had no atrial flutter. DCG results showed that average frequencies of atrial premature beats and PAF were 10998±1896 and 232±46 respectively. In all 48 patients with PAF, 40 cases of them (83.3%)were induced by atrial premature beats and the others(16.7%) were induced by paroxysmal atrial tachycardia. There were statistically significant differences (P<0.01) between atrial premature beats induced PAF and others PAF in the index ofthe cycle of ectopic beat of atrial premature beats, the cycle before atrial premature beats and the ratio. Conclusions As a non invasive examination, DCG can determine the initiating agent for PAF, which is an important tool for clinical therapy and observation.
