【摘要】
目的：探讨同期放化疗治疗中晚期(Ⅲ～ⅣA期)鼻咽癌的临床疗效、生存期及毒副反应。 方法：63例III～ⅣA期鼻咽癌患者随机分为DC组(n=32)和PF组(n=31)。放疗均采用方法相同的普通常规放疗。化疗DC组采用DC方案： 多西他赛 75mg/m2 静滴，d1，卡铂AUC为4～6(一般200-400 mg/m2)静滴，d1；PF组采用PF方案：顺铂25mg/m2 静滴，d1-3 ；5-FU 500mg/m2 静滴，d1-5。两方案均在放疗的第一天开始进行，28天为一个周期,共2个化疗周期。分别观察两组的临床疗效、生存期及毒副反应。结果：CR率DC组为93.8%，PF组为87.1%，两组比较差异无统计学意义(P>0.05)。1年无复发生存率和无远处转移生存率DC组分别为93.8%和96.9%，PF组分别为90.3%和93.6%，两组比较差异无统计学意义(P>0.05)。2年生存率DC组分别为87.5%，PF组分别为83.9%，两组比较差异无统计学意义(P>0.05)。Ⅲ～Ⅳ级胃肠道反应及口腔黏膜反应DC组明显低于PF组(P<0.05)。Ⅲ～Ⅳ级白细胞降低DC组高于PF组,但差异无统计学意义(P>0.05)。周围神经毒性、肝肾毒性等无明显统计学差异。结论：D C方案同期放化疗治疗中晚期鼻咽癌毒副反应低，具有良好的疗效和耐受性。

The clinical study of carboplatin and docetaxel combined with concurrent radiotherapy for advanced nasopharyngeal carcinoma

【Abstract】 ObjectiveTo evaluate the curative effects , survival time and side effects of carboplatin and docetaxel combined with concurrent radiotherapy for advanced nasopharyngeal carcinoma.Methods63 cases of 63 patients with advanced nasopharyngeal carcinoma were randomly divided into DC group (n = 32) and PF group (n = 31). Both groups were given the same radiotherapy in ordinary conformal method. Group DC were treated with DC scheme: docetaxel 75 mg/m2 ivgtt, d1; carboplatin, 200- 400 mg/m2 ivgtt, d1; Group PF were treated with PF scheme: cisplatin 25 mg/m2 ivgtt, d1-3; 5 –Fu 500 mg/m2ivgtt, d1-5. The two schemes are in the first day of the radiotherapy, 28 days for a cycle, a total of two chemotherapy cycles. The two groups were observed the clinical curative effect, and adverse reaction of survival.
