    摘 要 糖尿病是一种非常常见的慢性病，不仅给患者的日常生活带来很大影响。也给家庭、社会等带来比较沉重的负担。对于糖尿病，一方面需采取药物治疗，另一方面也需要自我管理及个体化管理。虽国内外很多糖尿病社区管理模式都取得了一定的效果，但是仍存在一定的问题。对于糖尿病的综合防治的发展形势依旧非常严峻，还需要给予进一步的关注。
