    摘 要 目的：观察儿科重症监护室(PICU)患儿的四肢肌肉萎缩随时间的变化趋势。方法：纳入2020年6月到9月在复旦大学附属儿科医院PICU患儿(2～14岁)22例，测量并记录患儿每周四肢围度变化，用线性混合模型分析2次四肢围度测量的变化趋势，并校正年龄、性别、体质指数和入院诊断疾病协变量，计算校正的β值和95%可信区间(CI)。结果：随着PICU住院天数增加，第二次测量患儿的四组肢体围度较第一次均有下降趋势。校正年龄、性别、体质指数和入院诊断疾病类型等因素后，PICU患儿四肢围度随时间延长有显著下降(P<0.001)。结论：患儿四肢围度的变化可以用于辅助判断肌肉萎缩程度，对早期发现肌肉萎缩并进行康复介入有临床应用价值。
