摘 要 目的：通过水通道蛋白4(AQP4)抗体阴性视神经炎临床与磁共振影像研究，探讨影响视神经的独立危险因素。方法：回顾性分析我院50例AQP4抗体阴性视神经炎患者的临床资料。结果：在基线水平上及治疗后的随访终点发现50例患者中分别有85、26只眼睛的最佳矫正视力低于0.1；急性期视神经炎患者核磁共振成像(MRI)有36例出现多节段受累；多元逐步回归分析发现，基线视力水平、视神经受累节段数量是影响视神经的独立危险因素(P<0.05)；分析随访资料发现，单相病程、疾病复发、转变为视神经脊髓炎谱系疾病分别为40例、9例、1例。结论：AQP4抗体阴性视神经炎病发率较高，预后视功能较差；易复发，但不易转变为视神经脊髓炎谱系疾病。

    ABSTRACT Objective： To discuss the independent risk factors affecting the optic nerve by clinical and magnetic resonance imaging study of aquaporin 4 (AQP4) antibody negative optic neuritis. Methods： The clinical data from 50 patients with AQP4 antibody negative optic neuritis in our hospital were retrospectively analyzed. Results： There were 85 and 26 eyes in 50 patients with optimal corrected visual acuity less than 0.1 at baseline and at the end of follow-up after treatment. Multiple segmental involvement was observed in 36 patients with acute optic neuritis by MRI. Multiple stepwise regression analysis showed that baseline visual acuity level and the number of involved optic nerve segments were the independent risk factors affecting the final optic nerve (P<0.05).
