摘 要 目的：探讨家庭医生团队指导下的体重管理对慢性心力衰竭(CHF)患者中体重监测依从性及再住院率的影响。方法：选取2019年6月-2020年5月在上海市普陀区宜川街道社区卫生服务中心就诊的256例CHF患者，采用随机数字表法分为干预组(128例)和对照组(128例)。对干预组患者开展家庭医生团队的体重管理指导及干预，对对照组给予常规健康宣教。结果：干预前，两组患者的体重监测依从性分布和再住院率差异均无统计学意义(均P>0.05)。管理1年后，干预组患者体重监测依从性分布和自我管理量表评分均高于对照组，再住院率低于对照组，差异均有统计学意义(P<0.05)。结论：家庭医生团队指导下的体重干预有助于提高患者体重监测的依从性，降低再住院率。
