四维超声骨骼模式在胎儿半椎体畸形中的应用价值(1)
摘要:目的 探讨二维超声结合四维超聲在胎儿半椎体畸形中的应用价值。方法 应用二维超声及四维超声骨骼模式对8例半椎体病例进行图像分析。结果 二维超声显示5例发生在腰段,3例发生在胸段;胎儿脊柱旁矢状切面显示病变处椎体骨化中心缺失或模糊,脊柱冠状切面显示病变处脊柱侧弯或成角;四维超声骨骼模式显示病变处椎体骨化中心小或缺失,对侧骨化中心代偿性增大,直观动态的显示脊柱侧弯或成角畸形,通过肋骨及脊柱的关系可以清晰的定位病变椎体,图像逼真。结论 四维超声骨骼模式能在二维超声诊断胎儿半椎体畸形的基础上提供更多病变椎体的诊断信息,具有重要的临床参考价值,是二维超声影像诊断的必要补充。

Abstract:Objective To investigate the diagnostic value of two-dimensional ultrasound combined with four-dimensional ultrasound in fetal hemivertebra.Methods Two dimensional ultrasound and four dimensional ultrasound image analysis of skeletal pattern in 8 cases of hemivertebra cases.Results Two dimensional ultrasound showed 5 cases in lumbar,3 cases occurred in the thoracic spine;fetal parasagittal plane display lesions vertebral ossification center is absent or vague,coronal surface display lesions of spinal scoliosis or angle;Four dimensional ultrasound skeletal lesions vertebral ossification center pattern showed small or absent,the ossification center side compensatory increase,intuitive display dynamic scoliosis or angular deformity,through the relationship between the ribs and spine vertebral lesions can clear positioning,lifelike images.Conclusion Ultrasound diagnostic information of four-dimensional skeleton model can provide more vertebral two dimensional ultrasonic diagnosis of fetal hemivertebra,has important clinical reference value,is a necessary complement to two-dimensional ultrasound imaging diagnosis.

胎儿半椎体畸形是一种少见的脊柱发育畸形,其预后较为复杂,产前超声诊断对了解病情及明确临床诊断,选择终止妊娠或产后早期治疗有重要意义[1]。文献报道[2-3]目前胎儿半椎体畸形多采用二维或三维超声检查,二维超声在诊断胎儿半椎体畸形存在空间立体感不强、逼真度差,病变椎体定位不准的缺点,而三维超声虽能显示立体图像,但是不能动态观察,特别是在胎儿体位忽然发生变化的情况下,四维超声不仅能直观的显示脊柱病变处骨化中心的情况,脊柱弯曲度的改变,病变椎体的定位,还能在实时图像上动态的调整角度,便于观察,本研究应用二维超声及四维超声骨骼模式对8例半椎体病例图像进行分析探讨。
1资料与方法
1.1一般资料

2014年3月~2016年3月在我院进行的胎儿系统超声检查,其中诊断半椎体畸形的有8例,胎龄19~26 w,孕妇年龄21~28岁,平均年龄(24.5±6.29)岁。7例在当地医院引产,尸检证实1例,引产后超声复查证实1例,其余电话随访均有外观畸形;1例产后复查证实。其中发生在脊柱腰段5例,胸段3例。
1.2方法
采用彩超GE-E8和GE-730,容积探头频率2~5 MHz。对孕妇进行经腹探查,仔细观察胎儿脊柱的生理弯曲、长度、表面皮肤覆盖情况,椎体及椎弓形态、大小、数目及骨化程度等。声束平面从胎儿的背部入射,做脊柱的正中矢状切面和旁矢状切面,在矢状切面的基础上,探头旋转90°获得脊柱横切面,探头移向胎儿的两侧从腋后线纵切胎儿脊柱,获得脊柱的冠状切面。二维发现异常的病例,在脊柱正中矢状切面上启动4D键,调整取样框大小并将取样框置于感兴趣部位,选好合适的扫查角度后,选择骨骼模式观察脊柱形态,观察过程中可进行角度、平面等调整,以达到最佳图像。对每幅满意图像进行存图。, 百拇医药(张丽莎 张玲懋 郝晓英 张丹 高燕)
