酶联免疫法与冷凝集试验在小儿支原体肺炎感染诊断中的应用价值分析(1)

     摘 要:目的 分析小兒支原体肺炎感染采用酶联免疫法与冷凝集试验的诊断价值。方法 选取我院2015年3月~2017年3月收治的支气管肺炎患儿100例作为此次研究对象,采取所有患儿清晨空腹静脉血,离心取血清后,分别采用酶联免疫法及冷凝集试验检测血清肺炎支原体抗体IgM水平,比较两种检测方法阳性检出率,以支原体培养为诊断“金标准”,对两种检测方式的特异性、灵敏性、准确率、阳性预测值进行评估。结果 酶联免疫法特异性90.75%较冷凝集试验高,误诊率为9.26%较冷凝集试验29.63%低,差异具有统计学意义(P<0.05);酶联免疫法敏感性84.78%低于冷凝集试验97.83%,准确度88.00%,漏诊率15.22%高于冷凝集试验83.00%,2.17%,差异无统计学意义(P>0.05)。结论 酶联免疫法与冷凝集试验均各有优势,其中冷凝集试验敏感性高,漏诊率低,酶联免疫法特异度高、误诊率较低,故临床可将两者联合检测,以提升小儿支原体肺炎感染阳性检出率,降低误诊及漏诊率,临床具有较高的应用价值。

    Analysis of the Value of Enzyme-linked Immunosorbent Assay and Condensate Agglutination Test in the Diagnosis of Mycoplasma Pneumoniae Infection in Children

    Abstract:Objective To analyze the diagnostic value of enzyme linked immunosorbent assay and condensate agglutination test for mycoplasma pneumonia infection in children.Methods In our hospital in March 2015 to March 2017,100 cases of children with bronchial pneumonia were selected as the research object,all the children had fasting venous blood in the early morning and centrifuged the serum,and then made a detection of serum mycoplasma pneumoniae antibody IgM level by enzyme-linked immunosorbent assay and condensation agglutination assay,the positive detection rate of the two methods was compared,and the specificity,sensitivity,accuracy and positive predictive value of the two methods were evaluated by using mycoplasma culture as the"gold standard"for diagnosis.Results The specificity of enzyme-linked immunosorbent assay 90.75% was higher than that of cold agglutination test,and the misdiagnosis rate 9.26% was lower than that of cold agglutination test 29.63%,the difference was statistically significant(P<0.05),the sensitivity of enzyme-linked immunosorbent assay was 84.78%,lower than that of cold agglutination test 97.83%,the accuracy rate was 88.00%,and the missed diagnosis rate 15.22% was higher than that of condensation test 83.00%,2.17%,the difference was not statistically significant(P>0.05).Conclusion Enzyme-linked immunosorbent assay and condensate agglutination test have their respective advantages,among them,the condensation assay has high sensitivity,low missed diagnosis rate,high specificity and low misdiagnosis rate.In order to increase the positive rate of mycoplasma pneumonia in children and reduce the rate of misdiagnosis and missed diagnosis,it has a high clinical value., http://www.100md.com(汤成玉)
