康复训练联合物理因子治疗肩袖损伤的临床疗效观察(1)
摘 要:目的 观察康复训练联合多种物理因子治疗肩袖损伤的临床效果。方法 选取我院2015年2月~2016年12月肩袖损伤患者60例,随机分为治疗组和对照组,各30例。对照组患者采用物理因子治疗,包括中频电治疗仪及超声治疗仪、超短波治疗仪;治疗组在对照组治疗基础上增加运动康复训练,采用Neer肩关节功能评分评估两组疗程结束后治疗效果。结果 两组治疗后Neer评分与治疗前相比均具有统计学意义(P<0.05);疗程结束后治疗组评分显著高于对照组[(83.20±2.23)分vs(93.26±1.46)分],组间差异有统计学意义(P<0.05)。结论 康复训练联合多种物理因子治疗肩袖损伤,可有效促进患者肩关节功能的恢复,提高临床疗效,值得临床推广使用多手段联合治疗肩袖损伤。

Abstract:Objective To observe the clinical effect of rehabilitation training combined with various physical factors in the treatment of rotator cuff injury.Methods 60 patients with rotator cuff injury in our hospital from February 2015 to December 2016 were randomly divided into treatment group and control group,with 30 cases in each group.The patients in the control group were treated with physical factors,including medium-frequency electric therapeutic apparatus and ultrasonic therapeutic apparatus and ultrashort wave therapeutic apparatus.The treatment group was given exercise rehabilitation training on the basis of the control group,and the Neer shoulder function score was used to evaluate the therapeutic effect after the end of treatment.Results The Neer score of the two groups after treatment was significantly higher than that before treatment(P<0.05).After the treatment,the score of the treatment group was significantly higher than that of the control grou[(83.20±2.23)vs(93.26±1.46)],there was significant difference between groups(P<0.05).Conclusion Rehabilitation training combined with a variety of physical factors in the treatment of rotator cuff injury can effectively promote the recovery of shoulder joint function and improve clinical curative effect.It is worth to promote the use of multiple approaches in clinical treatment of rotator cuff injury.

肩袖由四块肌肉构成:冈上、下肌,小圆肌及肩胛下肌,肩袖损伤常因四块肌腱出现损伤或无菌性炎症,表现为肩部疼痛、持续压痛、活动受限或肌肉萎缩、撕裂,好发于使用肩部频繁的人群[1]。从时间上分类主要分为急性损伤及慢性损伤。随着MRI的普及以及就诊意识提高,肩袖损伤诊断率越来越高,对于肩袖损伤的及时有效治疗显得尤为重要。肩袖损伤的治疗主要包括药物治疗、康复治疗和手术治疗。本研究运用康复训练联合物理因子疗法观察治疗肩袖损伤效果,报告如下。
1 资料与方法
1.1 一般资料 选取我院2015年2月~2016年12月门诊确诊的肩袖损伤患者60例。本次经我院伦理委员会批准,排除年龄>75岁,合并严重心肝肾等系统疾病者、精神病、肿瘤患者及不能坚持治疗者。患者签署知情同意书后随机分为治疗组与对照组,各30例。治疗组中男18例,女12例,年龄42~63岁,平均年龄(47.3±9.15)岁, 平均VAS评分(6.81±2.11)分;对照组男20例,女10例,年龄41~67岁,平均年龄(45.2±8.24)岁,平均VAS评分(7.51±1.77)分;各组患者的年龄、性别等比较,差异无统计学意义(P>0.05),具有可比性。
1.2方法
1.2.1对照组 采用物理因子治療,急性损伤72 h内制动休息。物理因子治疗采用YK-2000B型(广州一康科技有限公司)中频电治疗及达佳DL-C-BⅡ型超短波、CZT-8A型超声波治疗。中频治疗仪操作细节:患肩疼痛部位放置电极片,刺激量以肌肉收缩为度,20 min/次,1次/d,2 周为1疗程。超短波治疗法:患者卧位,患肩痛处前后对置电极,剂量为无热量,20 min/次,1次/d,2 周为1疗程。超声波疗法:患者坐位,患肩痛处涂抹耦合剂,探头往返微移稍用力按压,模式为连续式超声,强度为0.75~1.5 W/cm2,15 min /次,1次/d,2 周为1疗程[2]。, http://www.100md.com(陆鹏 杨小立 尚翠侠)
