急性前循环脑梗死患者梗死体积与血清NSE水平相关性研究(1)
摘 要:目的 探讨急性前循环脑梗死患者脑梗死体积与血清神经元特异性烯醇化酶水平的相关性。方法 收集2017年4月1日~9月30日我院收治的50~70岁的40例急性前循环脑梗死患者作为脑梗死组,另从我院查体中心随机抽取50例年龄、性别相匹配的健康志愿者做对照组,分别检测两组的血清初始NSE水平,并据颅脑磁共振DWI像计算脑梗死组患者的梗死体积。结果 脑梗死组血清初始NSE水平为(25.78±2.57)mmol/L,高于对照组的(7.96±0.23)mmol/L,统计学意义显著(P<0.001);颅脑磁共振的DWI相计算出的脑梗死体积与脑梗死组血清初始NSE水平呈高度相关(R=0.736,P<0.001)。结论 神经元损伤的定量生化标志物NSE,其水平高低可反映脑卒中后神经功能及脑组织损伤的严重程度,或许可作为急性脑梗死患者病情监测和预后评估的一项重要指标。

Correlation between Infarct Volume and Serum NSE Level in Patients with Acute Anterior Circulation Cerebral Infarction

Abstract:Objective To investigate the correlation between cerebral infarction volume and serum neuron specific enolase level in patients with acute anterior circulation cerebral infarction.Methods We collected 40 patients with acute anterior cerebral infarction from 50 to 70 years old who were admitted to our hospital from April 1st to September 30th,2017 as cerebral infarction group,and randomly selected 50 cases of age and gender matching from our physical examination center.The healthy volunteers were used as a control group.The serum NSE levels of the two groups were detected separately,and the infarct volume of the cerebral infarction group was calculated according to the brain magnetic resonance DWI image.Results The serum NSE level in cerebral infarction group was (25.78±2.57)mmol/L,which was higher than that in control group(7.96±0.23)mmol/L,statistically significant(P<0.001);The volume of cerebral infarction calculated from DWI of cranial magnetic resonance was highly correlated with the initial serum NSE level in cerebral infarction group(R=0.736,P<0.001).Conclusion The level of NSE,a quantitative biochemical marker of neuronal damage,can reflect the severity of neurological function and brain tissue damage after stroke,or it can be used as an important indicator of disease monitoring and prognosis evaluation in patients with acute cerebral infarction.

腦梗死(cerebral infarction)是临床上常见的多种原因导致的局部脑组织血液供应障碍,导致脑组织缺血缺氧性病变坏死,进而产生相对应的神经功能缺失表现,其具有发病率高、患病率高、致残率高、死亡率高的四大特点[1,2]。烯醇化酶普遍存在于各种生物体内,是细胞能量代谢过程中的关键酶,神经元特异性烯醇化酶(neuron specific enolase,NSE)水平可用来预测多种疾病的严重程度及预后,特别是中枢神经系统疾病[3]。本研究通过检测急性脑梗死患者早期(24 h内)血清的初始NSE值,并计算出颅脑磁共振的DWI此时对应的脑梗死体积,拟探讨二者之间的相关性,评价NSE水平在急性脑梗死患者病情评估及预后判断中的临床价值。, 百拇医药(单敏 常燕燕 赵大鹏)
