益胃活血方治疗血瘀型慢性萎缩性胃炎临床观察(1)
摘 要：目的 探讨益胃活血方治疗血瘀型慢性萎缩性胃炎的临床效果。方法 选取我院符合纳入标准的血瘀型慢性萎缩性胃炎患者46例，随机分成治疗组和对照组，每组23例。治疗组口服益胃活血方，对照组口服叶酸片，对比两组患者治疗前后的症状评分及病理组织学评分。结果 治疗组口服益胃活血方后对胃脘疼痛、胀满等主要症状的改善优于对照组，统计学意义显著(P<0.01)。经治疗后，两组患者的恶心呕吐、嗳气、反酸，纳差、乏力等症状积分均有所下降，治疗组效果优于对照组(P<0.05)；治疗组总有效率为91.30%，高于对照组的82.61%，差异具有统计学意义(P<0.05)。治疗后，两组患者的萎缩、肠上皮化生等病理组织学评分均较治疗前下降，且治疗组积分降低情况高于对照组(P<0.05)。结论 益胃活血方治疗慢性萎缩性胃炎，可改善慢性萎缩性胃炎的症状，改善胃粘膜萎缩、肠化等病理结果。

Clinical Observation on Yiwei Huoxue Recipe in Treating Chronic Atrophic Gastritis of Blood Stasis Type

Abstract：Objective To investigate the clinical effect of Yiwei Huoxue recipe in treating chronic atrophic gastritis of blood stasis type.Methods 46 patients with chronic atrophic gastritis of blood stasis type were randomly divided into treatment group and control group with 23 cases in each group.The treatment group was treated with Yiwei Huoxue recipe and the control group with folic acid tablet.The symptom scores and histopathological scores of the two groups were compared before and after treatment.Results After taking Yiwei Huoxue recipe，the treatment group improved the main symptoms of epigastric pain and bloating，which was better than that in the control group，the statistical significance was significant(P<0.01).After treatment，the scores of nausea and vomiting， belching，regurgitation，anorexia and fatigue in the two groups were all decreased，the effect of the treatment group was better than that of the control group(P<0.05)；the total effective rate of the treatment group was 91.30%，higher than that of the control group 82.61%， and the difference was statistically significant(P<0.05).After treatment，the histopathological scores of atrophy and intestinal metaplasia in the two groups were all lower than that before the treatment，and the score of the treatment group was lower than that of the control group(P<0.05).Conclusion Yiwei Huoxue recipe for chronic atrophic gastritis can improve the symptoms of chronic atrophic gastritis， gastric mucosal atrophy，intestinal metaplasia and other pathological results.

慢性萎缩性胃炎(chronic atrophic gastritis，CAG)是指胃粘膜上皮反复遭受各种攻击因子的侵袭，导致胃粘膜上皮固有腺体減少，常伴有肠上皮的化生和(或)上皮内瘤变的一种慢性胃部疾患。我国目前慢性萎缩性胃炎的发病率高达23.2%[1]。正常的胃粘膜从经过长期的、慢性炎症刺激，固有腺体发生萎缩，进一步出现肠上皮化生，发展成为粘膜的不典型增生，最后成为癌变的胃粘膜的病理演变，这是胃癌的病理发展模式[2]。所以，慢性萎缩性胃炎是胃癌前病变之一。我院运用益胃活血方治疗慢性萎缩性胃炎中胃络瘀阻证的患者，取得了满意的临床效果，现报道如下。, 百拇医药(陶剑 奚美娟 张平)
