308nm准分子激光联合氟米松软膏治疗局限性神经性皮炎临床观察(1)
摘 要：目的 评价308 nm准分子激光联合氟米松软膏治疗局限性神经性皮炎的临床疗效。方法 选取2016年11 月～2017年11月在黄石市中心医院皮肤科门诊诊断为局限性神经性皮炎患者90例，数字编码随机分为三组，每组30例，治疗组采用308 nm准分子激光联合氟米松软膏，对照组1采用308 nm准分子激光照射，对照组2外用氟米松软膏，对三组有效率、不良反应及复发率对比。结果 治疗组治疗2周、4周的有效率分别为63.33%、86.67%，高于对照组1的16.67%、63.33%，对照组2的36.67%、56.67%，差异有统计学意义(P<0.05)。治疗组的不良发生率为16.67%，低于对照组1、2的36.67%、66.67%，差异有统计学意义(P<0.05)。治疗组的复发率为6.67%，低于对照组1、2的60.00%、70.00%，差异有统计学意义(P<0.05)。结论 308 nm准分子激光联合氟米松软膏治疗局限性神经性皮炎起效快、疗效好、安全性好、复发率低。

Abstract：Objective To evaluate the clinical efficacy of 308 nm excimer laser combined with flumetasone ointment in the treatment of localized neurodermatitis.Methods From November 2016 to November 2017，90 patients with localized neurodermatitis diagnosed in the dermatology clinic of Huangshi Central Hospital were randomly divided into three groups，30 in each group.The treatment group used 308 nm excimer.The laser was combined with dexamethasone ointment，the control group 1 was irradiated with 308 nm excimer laser，and the control group 2 was treated with flumetasone ointment.A comparison of the three groups of efficacy，adverse reactions， and relapse rates.Results The effective rates of treatment group were 63.33% and 86.67% at 2 weeks and 4 weeks respectively， higher than 16.67% and 63.33% in control group 1，and 36.67% and 56.67% in control group 2，respectively，the difference was statistically significant(P<0.05).The incidence of adverse events in the treatment group was 16.67%，which was lower than that of the control group 1， 2 36.67%，66.67%，and the difference was statistically significant(P<0.05).The recurrence rate of the treatment group was 6.67%，which was lower than that of the control group 1， 2 60.00%，70.00%，and the difference was statistically significant(P<0.05). Conclusion 308nm excimer laser combined with flumetasone ointment for the treatment of localized neurodermatitis has a fast onset， good curative effect，good safety and low recurrence rate.

神經性皮炎(neurodermatitis)又称慢性单纯性苔藓，是以阵发性剧痒和皮肤苔藓样变为特征的慢性炎症性皮肤病，依其受累范围的大小可分为局限性及播散性[1]。本病病因尚不明确，虽治疗方法多样，但无特效治疗手段，瘙痒-搔抓-瘙痒的恶性循环严重影响生活质量。近年来308 nm准分子激光在皮肤科的应用越来越广泛，在白癜风、银屑病、慢性湿疹、斑秃等疾病中取得了较好的疗效[2-5]。查看文献发现其在治疗局限性神经性皮炎中的报道罕见，故为了解308 nm准分子激光对局限性神经性皮炎的疗效，本文于2016年11 月～2017年11月应用308 nm准分子激光联合氟米松治疗局限性神经性皮炎患者，结果汇报如下。
1资料与方法
1.1一般资料 选取2016年11 月～2017年11月在黄石市中心医院皮肤科门诊诊断为局限性神经性皮炎患者，本次研究经过医院伦理委员会批准。纳入标准：①符合局限性神经性皮炎诊断标准；②年龄18～65岁；③皮损分布于躯干、四肢及颈背部；④接受治疗前1 周内未服用或外用糖皮质激素，未服用过抗组胺药物；⑤自愿参加本临床研究并签署知情同意书。排除标准：①皮损发生于面部或外阴部或分布广泛者；②1周内曾外用或内服过其他糖皮质激素类及抗组胺药物者；③有光敏者；④有严重内科疾病或免疫功能低下者；⑤妊娠及哺乳期妇女。采用数字编码随机对照研究的方法， 将入选患者分为三组，每组30例。其中治疗组男性18例，女性12例，年龄19～60岁，平均年龄(42.32±15.14)岁，病程0.5～18年，平均病程(4.08±1.25)年。对照组1男性16例，女性14例，年龄22～58岁，平均年龄(41.25±14.56)岁，病程1～10年，平均病程(3.28±2.09)年。对照组2男性17例，女性13例，年龄20～62岁，平均年龄(43.08±13.12)岁，病程0.5～20年，平均病程(4.21±1.02)年。皮损主要分布于腰骶部、颈项及四肢伸侧，为多角形扁平丘疹或苔藓样改变。每位患者均有1～3个不同部位的皮损。两组患者在性别、年龄、病程及治疗前皮损严重程度，差异无统计学意义(P>0.05)，具有可比性。, http://www.100md.com(邹爱玲 石年)
