三维超声定量诊断不同类型子宫发育异常的准确性及应用价值(1)

     摘 要:目的 分析不同类型子宫发育异常以三维超声定量诊断的准确性、应用价值。 方法 以2015年8月~2018年2月本院产前检查为子宫发育异常可疑孕妇120例为研究对象,所有孕妇均先以二维彩超进行初次筛查,后行三维超声定量诊断,以最终确诊结果为金标准,比较二维、三维彩超对不同类型子宫发育异常检出率,分析两种检查方式对敏感性、特异性、准确性临床应用价值。结果 确诊正常14例,异常106例,三维共诊断107例患者,二维检查共诊断出111例;三维检查准确性、敏感性、特异性分别为99.17%、99.07%、100.00%,高于二维84.17%、 89.19%、22.22%,差异有统计学意义(P<0.05)。结论 临床对子宫发育异常者使用三维超声定量诊断,不同类型疾病诊断准确性高,有较高应用价值。

    Abstract:Objective To analyze the accuracy and application value of three-dimensional ultrasound quantitative diagnosis of different types of uterine dysplasia.Methods From August 2015 to February 2018,the prenatal examination of the hospital for 120 patients with suspected uterine dysplasia was studied.All pregnant women were first screened with two-dimensional color ultrasound,after three-dimensional ultrasound quantitative diagnosis,the final diagnosis result is the gold standard,compare the detection rate of different types of uterine dysplasia by two-dimensional and three-dimensional color Doppler ultrasound,and analyze the clinical application value of the two examination methods for sensitivity,specificity and accuracy.Results 14 cases were diagnosed as normal, 106 cases were abnormal,107 cases were diagnosed by three-dimensional co-diagnosis,and 111 cases were diagnosed by two-dimensional examination.The accuracy,sensitivity and specificity of the three-dimensional examination were 99.17%,99.07%,and 100.00%,respectively,which were higher than the two-dimensional 84.17%,89.19%,and 22.22%,the difference was statistically significant(P<0.05).Conclusion Clinically,three-dimensional ultrasound quantitative diagnosis is used for uterine dysplasia.Different types of diseases have high diagnostic accuracy and high application value.

    人体生殖器官主要在胚胎期经由胚胎组织发育演变形成,其中女性子宫发育主要是通过苗勒氏管发育、融合及中隔吸收形成,若在此期间受某些外界或自身因素影响,造成组织吸收、融合障碍,易导致子宫发育异常。相关数据统计显示子宫发育异常发生率在 0.1%~10%,仅通过妇科检查难以进行确诊,同时子宫发育异常类型繁多,这使得分型诊断难度上升、准确度降低[1]。目前子宫发育异常诊断方式有腹腔镜、物理检查、超声、宫腔镜等,其中超声具有无创、检查快捷、可重复性高等优点,是疾病诊断首选方式,但大量实践表明二维检查受多种因素影响存在一定漏诊、误诊情况[2]。本文通过观察2015年8月~2018年2月可疑子宫发育异常者120例三维超声定量诊断的情况,旨在为疾病的诊断提供依据。

    1资料与方法

    1.1一般资料 以广安市人民医院2015年8月~2018年2月产前检查为子宫发育异常可疑孕妇120例为本次研究对象。纳入标准:①已婚者有流产、不孕、经量过多等病史;②未婚者有月经异常病史;③自愿参与研究签署知情同意书。排除標准:①言语、精神障碍;②合并其它疾病者。患者年龄19~39岁,平均年龄(29.52±3.15)岁,体质指数18.92~32.67 kg/m2,平均体质指数(23.67±2.33)kg/m2,其中已婚96例,未婚24例。, 百拇医药(吴清)
