血清睾酮、雌二醇水平与女性尿失禁的相关性研究(1)
摘 要：目的 研究血清睾酮、雌二醇水平对女性尿失禁严重程度的影响。方法 回顾性分析2009年5月～2016年5月本院收治的264例女性尿失禁患者的临床资料，根据尿失禁发病特点，分为压力性尿失禁、急迫性尿失禁、混合性尿失禁，检测尿失禁患者血清睾酮、雌二醇水平，分析血清睾酮、雌二醇水平与女性尿失禁的相关性。结果 压力、急迫和混合性尿失禁女性的血清睾酮、雌二醇水平低于无尿失禁女性，差异有统计学意义(P<0.05)；压力、急迫和混合性尿失禁女性的血清睾酮、雌二醇水平比较，差异无统计学意义(P>0.05)。血清低睾酮和低雌二醇水平与女性压力性尿失禁呈正相关(r=0.22，0.27，P<0.05)。结论 血清睾酮、雌二醇水平低下的女性比正常女性尿失禁的发病率增加，血清睾酮、雌二醇水平低下与女性尿失禁增加有相关性。

Abstract：Objective To investigate the correlation of serum testosterone and estradiol levels with urinary incontinence in women. Methods 264 urinary incontinence in women were selected and divided into stress urinary incontinence， urge incontinence and mixed incontinence. The levels of serum testosterone and estradiol in patients with urinary incontinence were detected，the correlation between serum levels of testosterone and estradiol and female urinary incontinence was analyzed. Results The levels of serum testosterone and estradiol in women with stress， urgency and mixed incontinence were significantly lower than those in women without urinary incontinence， with statistical difference (P<0.05). There was no significant difference in serum testosterone and estradiol levels between women with stress， urgency and mixed urinary incontinence (P>0.05). Serum low testosterone and low estradiol levels were positively correlated with female stress urinary incontinence (r=0.22，0.27，P<0.05).Conclusion Women with low levels of serum testosterone and estradiol have an increased incidence of urinary incontinence compared with normal women， and low serum testosterone and estradiol levels are associated with increased urinary incontinence in women.

尿失禁(urinary incontinence)是由多種因素引起的不自主尿液漏出，对女性身体、心理和社交等产生很大的不良影响。成年女性中约33%存在尿失禁，并随着年龄增大发病率逐年增加[1]。由于创伤性损害、盆底肌肉萎缩、尿道括约肌麻痹、盆底支持组织中胶原蛋白含量异常等因素，使女性尿失禁发生的可能性增大[2]。动物实验研究表明，尿失禁发病与性激素水平改变有关，在人体有无类似作用尚不明确[3]。本次研究旨在明确血清睾酮、雌二醇水平和女性尿失禁之间是否有相关性。现报道如下。
1 资料与方法
1.1一般资料 选择2009年5月～2016年5月在河北省三河市燕郊人民医院就诊并有完整医疗记录的女性尿失禁患者264例。纳入标准：①尿失禁诊断参照《2014年美国医师协会临床实践指南：女性尿失禁的非手术治疗》[4]；②根据尿失禁发病特点，分为压力性尿失禁、急迫性尿失禁、混合性尿失禁。③经由指定的泌尿科专家进行临床评估。排除标准：①有神经系统疾病史；②尿路畸形、腰椎间盘疾病；③有影响下尿路功能的特殊用药史。另选择398名健康体检者纳入调查研究。, 百拇医药(沈新伟 庞春萍 谢建军)
