2型糖尿病周围神经病变与血清促甲状腺激素关系的研究(1)

     摘要:目的 分析2型糖尿病(T2DM)患者周圍神经病变与血清促甲状腺激素(TSH)水平的关系。方法 选择329例柳州市人民医院内分泌科住院T2DM患者进行横断面研究,按照有无神经病变分为糖尿病周围神经病变(DPN)组193例和无糖尿病周围神经病变(NDPN)组136例。收集所有患者临床资料,检测两组患者HbAlc、TC、TG、HDL-C、LDL-C、TSH水平。结果 两组HbAlc、TC、HDL-C、LDL-C、TSH组间比较,差异无统计学意义(P>0.05);DPN组年龄大于NDPN组,糖尿病病程长于NDPN组,统计学意义显著(P<0.01);DPN组TG水平低于NDPN组,差异有统计学意义(P<0.05)。Logistic回归分析发现,HbA1c、病程及年龄进入最终回归方程[OR=1.170,95%CI:1.048~1.305(HbA1c);OR=1.059,95%CI:1.013~1.108(病程);OR=1.043,95%CI:1.015~1.072(年龄)],HbA1c、病程及年龄是DPN发生的危险因素。结论 HbA1c、病程及年龄是DPN发生的危险因素,住院T2DM患者DPN与血清TSH水平无明显相关。

    Abstract:Objective To analyze the relationship between peripheral neuropathy and serum thyroid stimulating hormone (TSH) levels in patients with type 2 diabetes mellitus (T2DM). Methods A total of 329 patients with T2DM in the Department of Endocrinology, Liuzhou People's Hospital were selected for cross-sectional study. According to the presence or absence of neuropathy, 193 patients with diabetic peripheral neuropathy (DPN) and 136 patients without diabetic peripheral neuropathy (NDPN). The clinical data of all patients were collected and the levels of HbAlc, TC, TG, HDL-C, LDL-C and TSH were detected in the two groups. Results There was no significant difference between the two groups of HbAlc, TC, HDL-C, LDL-C and TSH (P>0.05);The age of DPN group was longer than that of NDPN group. The duration of diabetes was longer than that of NDPN group,the statistical significance was significant (P<0.01); the TG level in the DPN group was lower than that in the NDPN group,the difference was statistically significant (P<0.05). Logistic regression analysis found that HbA1c, disease duration and age entered the final regression equation [OR=1.170, 95%CI: 1.048~1.305 (HbA1c); OR=1.059, 95%CI: 1.013~1.108 (sick course); OR=1.043, 95 %CI: 1.015~1.072 (age)], HbA1c, duration of disease and age are risk factors for DPN. Conclusion HbA1c, duration of disease and age are risk factors for the development of DPN. There is no significant correlation between DPN and serum TSH levels in hospitalized T2DM patients.
