摘要：信息化技术在健康领域的发展，给慢性病管理带来了契机。本文通过介绍患者门户(PPS)这项信息化工具的国内外应用现状、发展趋势和作用、基于该系统的智慧慢病管理的内容和要求，分析我国慢病管理的现状和存在的问题，提出利于我国慢病管理的远程医疗服务模式和移动干预方案，以期为慢病信息化发展和防控提供参考。
