    【摘要】目的：探索全科-专科联合模式在糖尿病社区管理中的应用方法及应用效果。方法：本研究样本来源于怀化市鹤城区宏宇社区卫生服务中心慢病管理库，采取方便抽样法选取128例2型糖尿病患者入组研究，每组64例，对照组采用常规干预模式，观察组采用全科-专科联合模式。结果：观察组对应评价指标均优于对照组，差异具有统计学意义(P<0.05)。结论：全科-专科联合模式在2型糖尿病社区管理中应用效果良好，可有效提高糖尿病患者血糖管理效率，在糖尿病的社区防控中有着较高的应用效益。
