    【摘要】目的：总结门诊药房处方调剂差错原因及干预措施。方法：于2019年6月—2021年5月开展研究，选取门诊药房取药出现处方调剂差错的45例患者为研究对象，对差错原因进行分析，并提出干预措施。结果：门诊药房处方调剂差错种类包括审核差错、调剂差错及发药差错，分别占比44.4%、33.3%及22.2%。门诊药房出现处方调剂差错的原因包括临床医师因素、临床药师因素及客观因素，各占比22.2%、62.2%及15.6%。结论：对门诊药房处方调剂差错原因进行分析，并提出可行的干预措施，有助于降低差错率，提升服务质量。
