    【摘要】伴随着国民生活质量的不断提升，对健康愈发关注，但是传统社区的卫生管理已经没有办法较好的满足目前居民健康需求逐步提升，所以社区中的卫生管理工作需尽可能地满足老年人和慢性病居民的实际健康需求，探求最好的卫生管理方式。现阶段，社区卫生管理服务推进中，“医防融合”方式受到多方的关注，从维护健康作为中心，将家庭医生签约相关服务作为管理的切入点，切实的履行与满足基础医疗需求与公共卫生服务职能，给居民提供更具健康性的管理服务。制定出适宜和完善性的健康方案，确保卫生服务的工作可以朝着个性化方向不断发展，依照整体的服务需求，给居民提供更具针对性的健康宣教服务，使得居民对疾病的认知程度明显加深，提升其服务的依从性，给社会中卫生管理工作夯实基础。此种方式，要求专业医师按照科学的方式，从病理和生理角度上出发，给居民量身打造出健康生活的计划和方案，此种方式不但可提升其健康水平和身体素质，若是可以广泛地普及，还可以使得国民的健康程度有质的飞跃，减少全民医疗所需的费用。
