    【摘要】目的：研究家庭醫生签约和慢性病管理对社区高血压控制效果及其影响因素。方法：选取本中心收治的社区高血压患者80例，随机均分为实验组和对照组，两组患者均实行常规管理，对实验组施加家庭医生签约和慢性病管理模式，对比两组患者病情控制的效果，同时经验性地总结高血压控制效果相关影响因素。结果：就血压而言，干预前，两组患者数值对比无较大差异(P>0.05)；干预后，实验组的收缩压为(135.62±7.43)mmHg，舒张压为(85.22±4.26)mmHg，对照组的收缩压为(142.06±9.51)mmHg，舒张压为(89.67±5.06)mmHg，数值对比组间差异显著(P<0.05)；就健康知识评分而言，实验组为(92.13±6.07)分，对照组为(79.68±2.14)分，组间差异显著(P<0.05)；就治疗依从性而言，实验组为92.50%(37/40)，对照组为80.00%(32/40)，组间差异显著(P<0.05)。各项指标均为实验组优势明显。结论：临床上应该加强对高血压患者治疗工作的重视，可以施加家庭医生签约和慢性病管理的模式对患者起到持续性的指导及监督作用，提升生活质量并改善预后。
