    【摘要】目的：探討重组人粒细胞刺激因子(rhG-CSF)不同干预时机在胃癌化疗患者中的应用效果。方法：选取2020年5月—2021年3月在我院进行化疗治疗的胃癌患者78例，采用随机数字表法分为观察组与对照组，各39例。两组均给予常规治疗及rhG-CSF皮下注射，观察组在化疗结束后24h时皮下注射，对照组在白细胞计数(WBC)<4×109/L时给予皮下注射。比较两组化疗期间感染率、WBC最低值、粒细胞最低值、WBC低于正常值维持时间及治疗前后免疫功能(CD3+、CD4+、CD4+/CD8+与NK)水平。结果：观察组感染率为7.69%(3/39)低于对照组的25.64%(10/39)(P<0.05)；观察组WBC最低值低于对照组，WBC低于正常值维持时间短于对照组，粒细胞最低值高于对照组(P<0.05)；治疗后，两组CD3+、CD4+、CD4+/CD8+与NK水平均低于治疗前，观察组高于对照组(P<0.05)。结论：胃癌患者化疗后应用rhG-CSF能有效改善患者免疫功能，同时早期预防应用rhG-CSF能有效降低化疗感染率，减缓白细胞及粒细胞下降程度，时机适宜，效果显著。
