    【摘要】目的：研究社區高血压患者健康管理中结合家庭医生签约、微信平台健康教育的应用效果。方法：于2021年11月—2022年11月在我社区卫生服务中心随机抽选96例高血压患者为调查对象，依据双色球法划入两组，即研究组(微信平台健康教育结合家庭医生签约)和常规组(社区医生常规教育)各48例。比较两组管理结局。结果：相较于常规组，研究组管理后的TC、SBP、DBP指标均明显更小(P<0.05)。相较于常规组，研究组管理后的SF-36、自我管理能力评分均明显更高(P<0.05)。结论：对于社区高血压患者，结合应用家庭医生签约、微信平台健康教育，可为其提供更细致、优质的连续健康指导，提高血压控制效果与患者的自我管理能力，推广价值明显。
