【摘要】目的：探析家庭医生护理团队在参与社区居民慢病管理中的效果。方法：收集2022年1月—2023年1月社区管理的慢病患者共750例，使用电脑随机分组法将全部患者分成研究组和对比组两组，对比组行常规门诊管理，研究组行家庭医生护理团队管理，比对两组的指标。结果：研究组的血压指标和血糖指标低于对比组，比对有统计学差异(P<0.05)；研究组患者的血压控制率和糖尿病控制率高于对比组患者，比对有统计学差异(P<0.05)；在诊疗依从率和满意率对比中，研究组均高于对比组，比对有统计学差异(P<0.05)。结论：家庭医生护理团队在参与社区居民慢病管理中的应用效果理想，可给予广泛的推广及应用。
