【摘要】目的：探讨慢性病护理管理联合健康教育对2型糖尿病合并冠心病患者生活质量的改善效果。方法：选择2022年3月—2023年3月住院的50例2型糖尿病合并冠心病患者作为研究对象，采取抽签方式将入组的患者平均分成两组，每组各25例，对照组采取普通治疗，观察组采取个性化综合饮食治疗干预。结果：以SF-36量表进行评估，观察组患者的各项评分指标较高(P<0.05)。观察组治疗满意度显著高于对照组，差异有统计学意义(P<0.05)。结论：采用慢性病护理管理联合健康教育进行2型糖尿病合并冠心病治疗，可以显著地降低患者的血糖水平，是一种值得推广的治疗方法。

    【Abstract】Objective： To explore the effect of chronic care management combined with health education on quality of life in patients with type 2 diabetes mellitus and coronary heart disease. Methods： 50 patients with type 2 diabetes mellitus combined with CHD who were hospitalized from March 2022 to March 2023 were selected as the study objects， and the enrolled patients were divided into two groups by drawing lots， with 25 cases in each group.
