    【摘要】目的：分析社区家庭医生签约在2型糖尿病患者慢病管理中的效果。方法：随机抽取2021年9月6日—2022年8月11日在本中心治疗的100例2型糖尿病患者，依据自愿签约原则，以接受社区家庭医生签约的50例患者为观察组，未签约行常规随访管理的50例患者为对照组。比较两组管理效果。结果：观察组HbA1c和2hPG均显著低于对照组，P<0.05；观察组患者对体育锻炼、合理饮食及血糖监测三项指标的依从性均高于对照组，P<0.05，差异有统计学意义。结论：社区家庭医生签约在2型糖尿病患者慢病管理中具有非常重要的作用。

    【Abstract】Objective：To analyze the effect of community family doctor signing contract in chronic disease management of type 2 diabetes patients.Methods：100 patients with type 2 diabetes who were treated in the center from September 6，2021 to August 11，2022were randomly selected.According to the principle of voluntary signing，50 patients who received the contract from community family doctors were the observation group，and 50 patients who did not sign the contract and received routine follow-up management were the control group.The management effects of the two groups were compared.Results：The HbA1c and 2hPG in the observation group were significantly lower than those in the control group ......
