【摘要】目的：分析完全腹膜外腹腔镜疝修补术(TEP)与开放疝修补术对腹股沟斜疝老年患者术后应激、免疫功能及近期生活质量的影响。方法：将2021年6月—2023年6月期间我科治疗的185例腹股沟斜疝患者作为研究对象，采取随机数表法将患者分为常规组(n=93)与腔镜组(n=92)，常规组实施开放疝修补术，腔镜组实施完全腹膜外腹腔镜疝修补术，对比两组效果。结果：两组患者术前皮质醇、肾上腺素、NA等指标相似(P>0.05)；术后1d及3d，腔镜组患者皮质醇、肾上腺素以及NA均低于常规组(P<0.05)。两组患者术前CD4+、CD8+以及CD4+/CD8+三项指标相似(P>0.05)；术后1d及3d，腔镜组患者CD4+、CD8+以及CD4+/CD8+均低于常规组(P<0.05)。术后，腔镜组物质、躯体、社会、心理四方面的生活质量评分均高于常规组(P<0.05)。结论：TEP可以有效减少老年患者术后应激，恢复其免疫功能，并提升其生活质量，应进行进一步临床推广。
