【摘要】目的：探讨米非司酮联合腹腔镜子宫肌瘤剔除术治疗子宫肌瘤疗效及对卵巢功能及复发率的影响。方法：选取2020年1月—2022年6月该院明确诊断的86例多发性子宫肌瘤患者，按随机数表法分为两组，每组43例。手术组以腹腔镜子宫肌瘤剔除术治疗，米非司酮+手术组在腹腔镜子宫肌瘤剔除术后口服米非司酮。观察两组子宫体积、复发情况及不良反应情况，记录两组治疗前后卵巢功能[黄体生成激素(LH)、雌二醇(E2)及卵泡生成激素(FSH)]。结果：术后，两组患者子宫体积均有明显减小(P<0.05)，且米非司酮+手术组子宫体积小于手术组(P<0.05)；治疗后，两组E2、LH及FSH均有明显降低(P<0.05)，且米非司酮+手术组E2、LH及FSH低于手术组(P<0.05)；米非司酮+手术组术后1年内复发率低于手术组(P<0.05)；两组患者术后不良反应发生率差异不显著(P>0.05)。结论：腹腔镜下多发性子宫肌瘤剔除术后口服米非司酮治疗能够明显降低患者术后复发风险，改善卵巢功能，且无明显毒副作用。
