社区实施糖尿病健康管理小组的策略及效果分析

摘 要 目的：探讨在社区成立糖尿病健康管理小组对提高糖尿病疗效、减少并发症发生的促进作用。方法：选取本社区建立健康档案的142例糖尿病患者作为研究对象，在配合糖尿病治疗的基础上，成立糖尿病健康管理小组，由全科医生、护士进行全面综合管理。观察3、6、9、12个月后，患者的合理运动、饮食控制以及血糖的控制情况，并与管理小组成立前进行对比。结果：成立糖尿病健康管理小组后，患者能够合理运动和控制饮食，血糖明显降低，与此前对照，差异有统计学意义(P<0.05)。结论：糖尿病健康管理小组可以有效地改善糖尿病患者的血糖控制情况，延缓并发症的发生。
