环介导等温扩增技术在检测妊娠期女性沙眼衣原体感染中的应用(1)

    摘要目的:评价酶联免疫吸附试验(ELISA)和环介导等温扩增方法(loop-mediated isothermal amplification,LAMP)在检测妊娠期女性沙眼衣原体(Chlamydiae trachomatis,Ct)感染中的应用价值。方法:根据Genebank公布的Cpn基因序列设计6条LAMP引物(2条内引物,2条外引物,2条环引物),优化并建立LAMP检测体系。收治Ct感染患者80例,进行LAMP法检测Ct DNA,同时用ELISA检测Ct特异性脂多糖抗原。结果:80例患者中,ELISA检测出阳性46例(57.5%),而LAMP检测出阳性56例(70.0%),两者比较差异无统计学意义(P>0.05)。80例患者中有51例服用过大环内酯类抗生素,ELISA检测出阳性26例(50.9%),而LAMP检测出39例(76.5%),两者比较差异有统计学意义(P<0.05)。结论:LAMP检测泌尿生殖道Ct感染方法简便、快捷,尤其是对服用过抗生素的患儿也具有较高的检出率。

    Application of loop mediated isothermal amplification technology in the detection of women chlamydiae trachomatis infection during pregnancy

    AbstractObjective:To evaluate the application value of enzyme linked immunosorbent assay(ELISA) and loop mediated isothermal amplification(LAMP) in the detection of women chlamydiae trachomatis(Ct) infection during pregnancy.Methods:6 LAMP primers(2 inner primers,2 outer primers,2 loop primers) were designed according to the sequence of Cpn gene published by Genebank.LAMP detection system was optimized and established.80 cases with Ct were selected.Ct DNA was detected by LAMP. Ct specific fat polysaccharide antigen was detected by ELISA.Results:In 80 cases,ELISA detected that 46 cases(57.5%) were positive,while LAMP detected that 56 cases(70%) were positive,and there was no statistical significance(P>0.05).In 80 cases,51 cases were treated with macrolide antibiotics;ELISA detected that 26 cases(50.9%) were positive,while LAMP detected that 39 cases(76.5%) were positive,and there was statistical significance(P<0.05).Conclusion:LAMP detection of urogenital Ct infection method is simple and fast, especially it has higher detection rate for the children with taking antibiotics.

    环介导等温扩增法(loop-mediated isothermal amplification,LAMP)是一种敏感、特异、方便快捷的核酸扩增技术[1]。因其无需特殊设备,且具有PCR类似的灵敏度和特异性,日益受到临床重视[2]。沙眼衣原体(Chlamydiae trachomatis,Ct)是一类严格细胞内寄生的病原体,不仅可以引起沙眼,还与人类各种泌尿生殖道疾病相关,如非淋菌性尿道炎、阴道炎、宫颈炎、子宫内膜炎等[3];Ct还可通过母婴传染引起胎膜早破、早产、新生儿死亡等[4]。Ct感染初期无症状或症状较轻,常易被忽视而导致严重的并发症。本研究通过对比LAMP和酶联免疫吸附试验(ELISA)在检测妊娠期女性沙眼衣原体感染情况,分析LAMP在检测沙眼衣原体感染中的临床应用价值。
