    【Abstract】BackgroundThe global prevalence of type 2 diabetes melitus(T2DM)continues to rise，posing a significanpublichealthchalenge.Thesharedcare modelfordiabeteshasshownsubstantialclinicalvalueinmanagingT2DM. However，mostresearch focuses onthe short-term efects，whilelong-termoutcomesremain underexplored.Additionally， thereisalackoflongitudinalstudiesexamining therelationshipbetweenfollw-upfrequency，appactivitylevels，andmetabolic indicators in T2DMpatients.ObjectiveTo assess the impactofregularoutpatientfollow-upandoff-siteappactivityon the metabolic indicators of T2DM patients within the sharedcare model，andto identifyfactors influencing regularfollow-up， providingareference forpromoting regular folow-up inT2DM management.MethodsAtotalof 448 TDM patients managed inthesharedcareoutpatientclinicofatertiaryhospitalin GansuProvincefrom2O21 to2O23were included.Basedonclinical records，patients were divided into two groups： the regular follow-up group ( ε?1 visit every 3 months)and the non-regular follow-upgroup.Healthrecords fromthe previous 3years werereviewed ......
