铸瓷贴面修复伴牙本质暴露的前牙牙体缺损疗效观察(1)
[摘要]目的：评价铸瓷贴面修复牙本质暴露的缺损前牙的临床效果。方法：31例因外伤、龋坏等原因导致的伴牙本质暴露的前牙缺损患者，使用46个铸瓷贴面进行修复，瓷贴面修复后3个月、6个月、1年和2年复查，应用改良Ryge指标(美国公共健康协会的修正指标)分析评价其临床疗效。结果：2年的临床复查结果显示，46个瓷贴面完整存在，有1例发生折裂，1例出现脱落，2例瓷贴面修复的基牙在复查时有轻度的牙龈炎症。97%以上的铸瓷贴面具有良好的边缘适合性，牙周健康，只有个别基牙出现继发龋，患牙周围牙槽骨无吸收，修复效果满意。结论：对于伴牙本质暴露的前牙缺损进行铸瓷贴面修复是一种可行的修复方法，且具有良好的美观效果。

Abstract： Objective To evaluate the clinical effect of using porcelain laminate veneer to restore defected anterior tooth with dentin exposure. Methods Ceramic veneers was applied to 46 teeth of 31 patients whose anterior tooth suffer dental defect with dentin exposure resulted from truma and decay. The 46 tooth were reviewed after luting in 3 months and 6 months， 12 months and 24 months. Modified Ryge standard (revised standard of American public health association)was applied to evaluate the clinical curative effect. Results During 2 years of review， 44 ceramic veneers were intact. One veneer was partly fractured after one years and 1 veneers falling out，and Slight gingivitis was found in 2 tooth.Margine accurancy was over 97% after two years review， periodontal health，and porcelain veneer keep stable in color， Secondary caries has only been found in individual abutment.No obvious absorption of alveolar bone around dental， prosthodontics effect is satisfied. Conclusion ceramic veneer prosthesis is a reliable and ideal dental aesthetic Prosthesis applied to restore defected anterior tooth with dentin exposure.

近10余年来，瓷贴面在口腔修复中的应用日益广泛，应用树脂粘接剂将全瓷贴面粘固在基牙上，以恢复和改善前牙的形态与颜色[1]。相较全冠类修复，瓷贴面修复使用微创的牙体预备方式，不仅仅考虑到修复体的固位，也考虑到剩余牙体组织的抗力[2]。现代粘结技术的发展允许这种微创的牙体预备，较少依赖于机械固位，主要通过粘接获得固位。
前牙外伤或者龋坏所致的牙体缺损常累及牙本质层，造成一定程度的牙本质暴露。患者因牙本质暴露自觉有轻微的冷热刺激感，影响发音及美观，为临床上较为常见的一种牙体缺损类型。由于牙本质在组成和结构上与釉质不同，富含水分和有机物，因此牙本质粘接修复的难度远高于釉质[3]；瓷贴面主要依靠粘接固位，用于修复牙本质暴露的前牙缺损就会变得比较复杂，存在一定的难度与风险。本研究使用热压铸瓷贴面(二硅酸锂基)对31例患者的46颗前牙进行修复，获得了满意的治疗与美观效果。
1 材料和方法
1.1 研究对象：选择2012年1月-2015年1月在云南省第一人民医院口腔医学中心进行铸瓷贴面修复的31例患者的46顆前牙作为研究对象。患者年龄18～46岁，平均为32岁，其中18例为单颗前牙的瓷贴面，8例为两颗前牙的瓷贴面，4例为三颗前牙的瓷贴面。
纳入标准：①前牙牙体缺损小于1/3，伴有唇面牙本质暴露；②患牙无明显松动，如有松动，行松牙固定术后再行修复；③患牙牙周组织正常，牙槽骨无病理性吸收，牙周膜间隙正常；④X线显示牙根无根折、根尖暗影及病理性吸收，死髓牙已经经过完善的根管治疗。所有患者咬合关系基本正常，前牙列完整，无明显的倾斜扭转牙。, http://www.100md.com(包凡 郭慧 董菲 汤雁利)
