羊膜移植联合广泛切除术治疗翼状胬肉的疗效分析(1)

     [摘要]目的：分析羊膜移植联合广泛翼状胬肉切除术治疗翼状胬肉的美容及预后效果。方法：选择2013年3月-2017年3月笔者医院收治的80例翼状胬肉患者为研究对象，随机分为观察组和对照组，每组40例。其中对照组41眼，采用单纯广泛翼状胬肉切除术治疗；而观察组43眼，在对照组基础上，给予羊膜移植术治疗。比较两组患者治疗效果、护理满意度及并发症发生情况等。结果：观察组治愈率为95.3%明显高于对照组的53.7%，而复发率(4.7%)显著低于对照组(46.3%)，差异具有统计学意义(P<0.05)。观察组总满意度为92.5%显著高于对照组的77.5%，差异有统计学意义(P<0.05)。观察组并发症发生率(巩膜溶解软化、巩膜坏死)明显低于对照组，差异具有统计学意义(P<0.05)。结论：羊膜移植联合广泛翼状胬肉切除术治疗翼状胬肉效果显著，可大大提高护理满意度，改善患者美容效果，在临床中值得应用推广。

    Abstract： Objective To study beauty and prognosis effect in pterygium patients by amniotic membrane transplantation combined with extensive pterygium resection. Methods 80 cases of patients with pterygium were divided into the observation group (40 cases) and the control group (40 cases) from March 2013 to March 2017. The control group was given simple extensive pterygium excision， while the observation group was received amniotic membrane transplantation. Treatment effect， nursing satisfaction and beauty effect of two groups were compared. Results The cure rate of the observation group was 95.3%， which was significantly higher than that of the control group (53.7%)， while the recurrence rate (4.7%) was significantly lower than that of the control group (46.3%)， and the differences were statistically significant(P<0.05). The total satisfaction rate of the observation group was 92.5%， which was significantly higher than that of the control group (77.5%， P<0.05). The incidence of complications (scleral lysis and scleral necrosis) in the observation group was significantly lower than that in the control group， the difference was statistically significant(P<0.05). Conclusion Amniotic membrane transplantation combined with pterygium excision is very effective in the treatment of pterygium， which can greatly improve the satisfaction of nursing and improve the cosmetic effect of the patients. It is worthy of application in clinical practice.

    翼狀胬肉是眼科常见疾病之一，由于其严重影响眼球活动、视力及美观，所以引起人们越来越多的重视[1]。目前，临床治疗常采用传统手术(单纯切除翼状胬肉)为主，但此方法常常不能彻底治疗眼部疾病，其术后复发率可达80%[2]。为防止传统手术高复发现象，近年来，羊膜移植术联合广泛翼状胬肉切除术被广泛应用于临床中[3-4]。由于其可明显降低复发率，提高治愈率，所以深受医疗机构青睐[5]。本研究对笔者医院收治的80例翼状胬肉患者临床资料进行分析，观察其治疗及美容效果，为临床提供治疗建议，现报道如下。

    1 资料和方法

    1.1 一般资料：回顾性分析2013年3月-2017年3月笔者医院收治的80例翼状胬肉患者的诊疗情况，按照治疗方式的不同分为观察组和对照组。观察组40例(43眼)，男25例，女15例；年龄27～67岁，平均(30.2±2.2)岁；病程3～5年，平均(3.4±1.5)年；初发性16眼，复发性27眼；对照组40例(41眼)，男性26例，女性14例；年龄26～67岁，平均(31.2±2.1)岁；病程3～6年，平均(3.7±1.2)年；初发性15眼，复发性26眼。患者胬肉侵入角膜缘内范围均为2～5mm。整个研究均在患者的知情同意下进行，并经过医院伦理委员会批准。两组患者在性别、年龄、病程等一般资料比较，差异不具有统计学意义(P>0.05)，具有可比性。(孙玉亮 张云 王斌)
