压电骨皮质切开术加速正畸牙移动的临床效果研究(1)
[摘要]目的:探讨压电骨皮质切开术即Piezocision技术在解除下牙列拥挤病例中的疗效。方法:選择需减数四颗第一前磨牙进行矫正治疗的错牙合畸形患者20例,平均分为Piezocision组10例和传统正畸组10例,对两组下牙列拥挤的排齐时间、牙根吸收及术后的疼痛不适进行对比,使SPSS 16.0统计软件进行数据分析。结果:Piezocision组下牙列拥挤解除所需时间(93.10±10.20)d较传统矫正疗程(120.30±8.01)d短,差异具有统计学意义(P<0.05),牙列排齐前后两组牙根长度均无明显吸收,差异无统计学意义(P>0.05),Piezocision组术后1周内患者诉疼痛不适和术区肿胀,但不影响正常生活。结论:Piezocision技术能够加速解除下牙列拥挤,且不伴发牙根吸收发生的风险,患者对该技术在治疗中应用的接受度较高。

Abstract: Objective To evaluate the efficiency of Piezocision(a minimally invasive corticotomy)assisted orthodontics in relieving the mandibular crowding. Methods 20 patients with malocclusion requiring orthodontic treatment by extracting 4 premolars were selected and divided into two groups: Piezocision group and traditional orthodontic group.Time needed for alignment, root resorption and postoperative discomfort were compared, SPSS 16.0 statistical software were usd for data analysis. Results In Piezocision group, the time needed to relieve the crowding of mandibular dentition was shorter than that of traditional orthodontic treatment, the difference was statistically significant, P<0.05. After dentition alignment,the length of rootin both group showed no statistically significant, P>0.05. The Piezocision group patients complained of slightly pain and intraoperative swelling, within 1 week after surgery but does not affect normal life. Conclusion Piezocision technology accelerates the alleviating of mandibular crowding and reduces the risks of root resorption compared with traditional orthodontic.Meanwhile the patients showed positive acceptance of the Piezocision in the treatment.

传统正畸通常需要2年左右,较长的治疗周期增加了牙周疾病、龋齿、牙根吸收发生的概率,也导致临床上一部分患者的就诊意愿降低。近百年来,借助牙槽外科手术达到加速牙齿移动从而缩短正畸疗程的研究一直在不断的进行,目前国内外学者对于骨皮质切开术加速正畸牙齿移动的疗效较为肯定[1-8],然而手术治疗的创伤性使得该方法在临床上的应用有限。2009年Dibar等[9-11]学者提出一种微创的骨皮质切开方法,即压电骨皮质切开术Piezocision,其为微创手术,无需翻瓣,大大降低术中及术后并发症。本实验对Piezocision在拔牙矫治病例中解除下牙列拥挤的疗效进行研究,以期为临床应用提供相关依据。
1 资料和方法
1.1 一般资料:选择2017年7月-2017年9月正畸科就诊的20例错牙合畸形患者为研究对象,分为实验组10例,年龄16~24岁,平均年龄(18.6±2.3)岁;对照组10例,年龄15~23岁,平均年龄(17.9±2.1)岁。实验组行Piezocision,对照组常规矫治。纳入标准:①年龄15~24岁错牙合畸形患者;②口腔卫生、牙周状况良好;③无局麻禁忌证;④无颅颌面畸形,无唇裂、腭裂,无颌面部创伤病史;⑤牙列完整无缺失,下前牙无牙体及牙周治疗史;⑥下牙列拥挤6~8mm;⑦需拔除四个象限第一前磨牙行正畸治疗;⑧下颌平面角为均角(31.1±5.6)°;⑨矫治过程中无需其他口内口外装置辅助解除拥挤。排除标准:①全身系统性疾病及骨代谢疾病患者;②长期服用影响牙齿移动药物的患者;③下前牙个别缺失;④行二次固定正畸的患者。
1.2 矫正装置:实验组与对照组均采用Damon Q自锁托槽矫治器,矫治弓丝:0.014、0.018、0.014×0.025英寸铜镍钛卵圆型圆丝、方丝,每根弓丝视牙列排齐情况使用4~8周。, http://www.100md.com(艾琦 王银龙 徐建光)
