Roy适应模式对鼻综合整形患者术后睡眠水平的影响(1)
[摘要]目的：探究Roy适应模式对鼻综合整形患者术后睡眠水平的影响。方法：选取2016年1月-2017年1月笔者医院收治的鼻综合整形手术患者91例，依据随机数字表法随机分为观察组45例和对照组46例。对照组给予常规心理护理及健康教育，观察组在对照组基础上给予Roy适应模式干预。对两组患者PSQI评分及护理满意度进行比较。结果：观察组住院时间、平均干预次数均低于对照组，差异具有统计学意义(P<0.05)。干预后，两组患者除入睡时间无明显差异，观察组其他指标优于对照组，差异均有统计学意义(P<0.05)；观察组护理满意度明显高于对照组，差异有统计学意义(P<0.05)。结论：Roy适应模式可有效改善鼻综合整形患者术后睡眠质量，缩短患者住院时间，提高患者护理满意度。

Abstract： Objective To investigate the effect of Roy-adaptive mode on postoperative sleep quality in patients with integrated nasal plastic surgery and to reduce the pain of postoperative sleep disorders. Methods 91 cases with nasal comprehensive plastic surgery in our hospital from January 2016 to January 2017 were selected and randomly divided into 45 cases in the observation group and 46 cases in the control group according to the random digital table method. The patients in the control group were given routine psychological care and health education. The observation group were given Roy-adaptive mode intervention on the basis of the control group. The general data of the two groups of patients， PSQI score and patient care satisfaction were compared. Results The hospitalization time and average intervention times in the observation group were lower than those in the control group， the differences were statistically significant(P<0.05). After intervention， there was no significant difference in sleep time between the two groups， but the other indexes in the observation group were better than those in the control group(P<0.05). The nursing satisfaction of the observation group was significantly higher than that of the control group(P<0.05). Conclusion Roy-adaptive mode can effectively improve sleep quality of patients with rhinoplasty， reduce the hospitalization time of patients and improve patient's satisfaction.

鼻綜合整形术是一种鼻整形术，其目的是对鼻子内部结构进行综合塑造，其有局部麻醉镇痛效果确切、手术刺激小的特点，但手术时间长、术后疼痛等因素使鼻综合整形患者术后易发生睡眠障碍，严重影响患者的术后康复速度，故对鼻综合整形患者术后睡眠障碍问题的关注成为护理工作中的一个重要内容[1-3]。Roy适应模式是于1964年由美国护理学家Sister Callista Roy提出，其探索中心为个体，从自我概念、生理功能、角色功能及相互依赖，对人作为一个适应系统对环境中各种刺激的适应性反应进行了重点探索[4-5]。近年来，Roy适应模式在多种疾病的护理中发挥了重要作用，但临床有关Roy适应模式对鼻综合整形患者术后睡眠水平的影响的相关研究甚少，故本研究对Roy适应模式对鼻综合整形患者术后睡眠水平的影响进行了探究。

1 临床资料
选取2016年1月-2017年1月笔者医院收治的鼻综合整形手术患者91例，其中男35例，女56例；年龄20～40岁，平均年龄(25.35±3.45)岁；美容整形76例，外伤整形15例。依据随机数字表法随机分为观察组45例和对照组46例。观察组：男17例，女28例，年龄21～40岁，平均年龄(25.45±3.85)岁，美容整形40例，美容整形5例；对照组：男18例，女28例，年龄20～39岁，平均年龄(24.42±3.29)岁，美容整形36例，外伤整形10例。纳入标准：患者有交流及阅读能力。排除标准：有其他导致睡眠障碍的严重疾病的患者。两组患者年龄、性别等资料比较，差异无统计学意义(P>0.05)。本研究经医院伦理委员会同意，患者及家属均知情同意并签署知情同意书。, 百拇医药(严涛 包香兰 陈卫华)
