一种矫治乳前牙反牙合的下颌联冠牙合垫斜面导板矫治器的制作和应用(1)
[摘要]目的：研制一种矫治乳前牙反牙合的下颌全牙弓联冠牙合垫附斜面导板矫治器，用于治疗因乳牙冠短小或依从性不良患者乳前牙牙性、功能性和轻度骨性反牙合畸形，以提高临床效果。方法：病例纳入标准为乳牙期因哺乳姿势不当、不良习惯、乳尖牙磨耗不足及遗传等造成的乳前牙牙性、功能性和轻度骨性反牙合畸形患者，共有23例佩戴研制的下颌全牙弓联冠牙合垫斜面导板矫治器10个月，拍治疗前后头颅定位侧位片，并用Steiner和Tweed方法分析测量治疗前后X线测量数据，并用SPSS 18.0软件配对t检验统计学处理。结果：SNA(°)、U1-NA (°)与治疗前相比，差异具有统计学意义(P<0.05)；ANB(°)、Wit’s(mm)与治疗前相比，差异具有显著统计学意义(P<0.01)。结论：该矫治器佩戴时可进食，不影响语言发音，固位良好、舒适、依从性良好；矫正了因哺乳姿势不当、不良习惯、乳尖牙磨耗不足及遗传等造成的乳牙期AngleⅢ类前牙牙性、功能性和轻度骨性反牙合畸形，对乳前牙反覆牙合深者疗效显著，提高了临床疗效。

Abstract： Objective To develop an orthodontic appliance for the treatment of anterior crossbite in anterior deciduous teeth， functional and mild skeletal deformity in patients with short laced crown or patients with poor compliance in order to improve clinical results. Methods The cases were included in the patients with anterior deciduous tooth， functional and mild skeletal deformity caused by improper lactation posture， bad habit， inadequacy of abutment wear and heredity， and 23 patients were worn for 10 months， and the head position before and after treatment was taken. Steiner and Tweed methods were used to analyze and measure X ray data before and after treatment. SPSS 18.0 software was used to match t test and statistical analysis. Results They had significant statistical significance P<0.05， SNA (degree)， U1-NA (degree)； there was very significant statistically significant P<0.01， ANB (degree)， Wit’s (mm). Conclusion Patients wearing this appliance can eat without be affected the language.It has good retention， comfortable and good compliance， and correction the teeth， functional and mild bone crossbite caused by improper nursing posture， bad habits， lactation tooth wear and heredity， and the depth of the recline teeth of the anterior teeth. The effect is remarkable and the clinical effect is improved.

我國乳前牙反牙合发病率为12.02%，其中乳尖牙磨耗不足而引起咬合干扰、不良喂养和不良习惯等是引起乳前牙牙性、功能性和骨性反牙合的主要原因，现有矫治方法有舌板撬动法、乳牙调磨法、下颌斜面导板、双曲舌簧牙合垫式矫治器、功能性矫治器FRⅢ等[1]。常常因为乳牙冠短小、矫治器固位不良、矫治器过大不舒适或患者依从性不良等原因，放弃治疗而影响治疗效果。鉴于此，作者研制了矫治乳前牙反牙合的下颌全牙弓联冠牙合垫附斜面导板矫治器，已应用临床近20年，治疗因乳牙冠短小或患者依从性不良的乳前牙牙性、功能性和轻度骨性反牙合畸形，效果良好，可临床推广应用。
1 资料和方法
