3D打印模型在颧弓骨折治疗中的应用效果及预后研究(1)

     [摘要]目的:探讨3D打印模型在颧弓骨折治疗中的应用效果及预后研究。方法:选择2016年3月-2019年3月于笔者医院进行治疗的80例颧弓骨折患者为研究对象,随机分为观察组(n=42)和对照组(n=38)。观察组采用3D打印模型联合局部小切口进行治疗;对照组采用局部小切口治疗,比较两组患者术中出血量、手术时间、住院时间、视觉模拟评分(VAS)、满意度及预后情况。结果:观察组患者术中出血量、手术时间、住院时间及VAS评分[(31.25±3.78)ml、(71.25±9.01)min、(1.63±0.51)d、(2.76±0.71)分]均高于对照组[(87.51±17.33)ml、(86.61±8.20)min、(3.26±0.45)d、(5.51±0.93)分](P<0.05)];观察组患者总满意度为97.62%,显著高于对照组的78.96%(P<0.05);术后随访,观察组对位准确率显著高于对照组,观察组患者张口受限、面部塌陷及咬合不良均显著低于对照组(P<0.05)。结论:在颧弓骨折患者中应用3D打印模型效果显著,可有效改善患者满意度及预后恢复情况,值得临床推广应用。

    Abstract: Objective To study Application effect and prognosis of 3D printed model in the treatment of zygomatic arch fracture. Methods 80 patients with zygomatic arch fractures who were treated in our hospital from March 2016 to March 2019 were randomly divided into the observation group (n=42) and the control group (n=38). The observation group was treated with a 3D printed model combined with a small local incision. The control group was treated with a small local incision. Intraoperative blood loss, operation time, hospital stay, visual analogue score (VAS), satisfaction and prognosis were compared between the two groups. Results The intraoperative blood loss, operation time, hospital stay and VAS score[(31.25±3.78)ml, (71.25±9.01)min, (1.63±0.51)d,(2.76±0.71)min] in the observation group were higher than those in the control group [(87.51±17.33) ml, (86.61±8.20)min,(3.26±0.45)d, (5.51±0.93)min](P<0.05). The total satisfaction of patients in the observation group was 97.62%, significantly higher than that in the control group (78.96%) (P<0.05). Postoperative follow-up showed that the alignment accuracy of the observation group was significantly higher than that of the control group, and the mouth opening limitation, facial collapse and poor occlusion of the observation group were significantly lower than that of the control group (P<0.05). Conclusion The application of 3D printing model in patients with zygomatic arch fracture has a significant effect and can effectively improve patient satisfaction and prognosis recovery, which is worthy of promotion and application.

    颧弓是面侧部较为突出的部分,容易受撞击而发生骨折,是颅颌面部常见的骨折类型,颧弓骨折,骨折端易发生錯位,临床表现为张口受限、眼睑闭合不全等,且骨折端发生错位在影响面容的同时还可因压迫下方组织而影响咬合功能,严重影响患者的生活质量[1-2]。临床使用局部小切口治疗该病,可利用切口进行分离至骨折处后作复位内固定,有效保护神经,但是受视野限制,常使复位不足,复位困难[3-4]]。随着微创技术的不断发展,3D打印技术已广泛应用于医学各个领域,且设计更加精确化,有独特的优势[5]。3D打印技术使实体立体模型得以制备,使外科医生获得清晰准确的图像信息,使患者更加直观地了解病情和治疗方案[6]。本研究旨在探讨3D打印模型在颧弓骨折治疗中的应用效果,并观察其对预后的影响,现将结果报道如下。, http://www.100md.com(张晓燕 于秀莉 蓝孝全)
