[摘要]目的：探讨联合筋膜鞘悬吊(Conjoint fascial sheath，CFS)+上睑提肌(Levator palpebrae superioris，LPS)复合体无张力缩短矫正术治疗重度上睑下垂的临床疗效。方法：收集2021年3月-2023年3月笔者医院80例重度上睑下垂患者，随机分为观察组(n=40)和对照组(n=40)。对照组行CFS+LPS复合体缩短矫正术，观察组行CFS+LPS复合体无张力缩短矫正术。比较两组术后1、3、6个月角膜中央反光点至上睑缘的距离(MRDI)、上睑上提长度、眼睑闭合不全量。比较两组术后1、3、6个月的临床疗效及并发症发生情况。结果：两组术后1、3、6个月的疗效差异无统计学意义(P>0.05)。两组患者的MRD1、上睑上提长度、眼睑闭合不全量指标组间、交互作用比较，差异无统计学意义(P>0.05)，而时间比较差异有统计学意义(P<0.05)。治疗后，观察组术后1个月、3个月并发症低于对照组(P<0.05)，两组术后6个月并发症差异无统计学意义(P>0.05)。结论：施行CFS+LPS复合体无张力缩短矫正术可有效治疗重度眼睑下垂，并可减少早期并发症的发生。
