应用Forsus矫治器后治疗安氏Ⅱ类错合下颌后缩的软硬组织的改变(1)

【摘要】目的：观察Forsus矫治器治疗安氏Ⅱ类错合下颌后缩患者颅颌面部软硬组织的改变。方法：选择辽宁医学院附属二院正畸科的恒牙早期安氏Ⅱ类错合畸形患者15例，男7例，女8例，年龄11—13岁，平均11.8岁。接受Forsus治疗的恒牙早期安氏Ⅱ类下颌后缩患者17例，治疗前后进行X线头影测量分析。结果：通过X线头影测量分析显示：SNA轻度减少，SNB增加，ANB减少，差异有显著性，，从而使上下颌间关系更为协调；U1-NA，，差异极具显著性，上切牙内收明显L1-NB增大，说明下切牙有少许唇倾。Pg’-N’-S增大有显著性：Ls-E、Li-E、Si-LiPg’、N’-Sn-Pg’减小，有显著性。
结论：Forsus矫治器对安氏Ⅱ类错合下颌后缩患者有矫形作用，可以促进下颌生长，抑制上颌生长，同时使得上下颌牙及牙槽骨发生改变，从而协调上下颌关系，改善面型。

Effects of Forsus appliance on class Ⅱ malocclusion with retrognathic mandible

【Abstract】OBJECTIVE:To assess retrognathic mandible changes caused by Class Ⅱ in the patients treated withForsus appliance during early permanent dentition. METHODS: A total of 15 adolescent patients，7males and 8females, aged 11.8 years (range 11～13) with class Ⅱ malocclusion were selected from Department of orthodontics,AffiliatedLiaoning Medical University of them,17 were treated with Forsus appliance as the control group.Cephalonetric analysis was used to compare the changes before and after treatment.RESULTS: The skeletal changes consisted of the increase in SNB and ANB. Dental alterations comprised retrusion of the upper incisors and labial tipping of the lower incisors. CONCLUSION:Forsus functional appliance possesses the orthopedic effect on AngleⅡmalocclusion.when repositioning the mandible forward and improving the growth of mandib,it creates the force to limit maxillary growth forward as well.Simultaneously it causes the changes of dento-alveolar. All these aspects contribute to a harmonious relationship of upper and lower jaws and a better profi.

安氏Ⅱ类错合最明显的特征是下颌后缩，治疗的重点主要是下颌骨的改型，这类患者往往需要使用功能性矫治器。Forsus矫治器(见图1)就是由美国正畸医师Bill Vogt在2001年设计制作的[1 2]，它是在Herbst矫治器基础上发展起来的一种新的固定的功能性矫治器，与固定矫治器相结合，具有独特的优越性。本实验选择了一组使用Forsus的安氏Ⅱ类错合畸形下颌后缩患者进行术前术后头影测量分析，探讨Forsus矫治器对颅颌面软硬组织的改变，评价Forsus矫治器对颅颌面软硬组织的矫治效果，为安氏Ⅱ类错合畸形下颌后缩的早期功能矫治提供依据。
