超声诊断卵巢卵黄囊瘤伴巧克力囊肿的表现及临床价值分析(1)
【摘要】目的：探讨卵巢的内胚窦瘤伴发巧克力囊肿的超声检查声像图特征性的表现，以提高超声对其诊断的准确率。方法：回顾性的分析2003年3月至2014年11月期间于我院经手术以及病理确认为卵巢的卵黄囊瘤伴发巧克力囊肿的37例超声资料，探讨其超声图像表现，同时与手术以及病理检查的结果进行对比，以分析其临床诊断价值。结果：37例的研究病历中，13例呈现实质性的回声，局部还可以见到蜂窝样的改变，22例呈囊实性的改变以及2例呈现囊性多房，同时实质部分还可以检出丰富的血流信号；此外，巧克力囊肿内部回声多样化，依据病程长短以及月经周期而不同，具有一定的特征性；超声对于卵黄囊瘤伴巧克力囊肿的诊断正确率达到86.5%。结论：卵巢的卵黄囊瘤伴发巧克力囊肿的超声图像具有一定的特征性，了解和认识卵黄囊瘤以及巧克力囊肿的超声图像特点，结合患者的年龄、血清中甲胎蛋白的水平等可提高相应的诊断率，对于临床诊断具有重要价值。

【Abstract】Objectives： To explore the characteristic performance of ultrasound image about ovarian yolk sac tumor and chocolate cyst in ovary， in order to improve the diagnosis accuracy. Methods： Retrospective analysis of the ultrasound data of 37 patients confirmed by surgery and pathology of ovarian yolk sac tumor and chocolate cyst in our hospital from March 2003 to November 2014 was conducted to investigate its performance of ultrasound images. Comparison with the surgical and pathological results was carried out to analyze its clinical diagnostic value. Results： In the 37 cases， 13 cases presented substantial echo with local cellular change； 22 cases had cystic changes and 2 cases presented cystic multilocular. Rich blood flow signals can also be detected in the substantial portion. Besides， Chocolate cyst’s internal echo was diverse and varied on the basis of duration of disease and the menstrual cycle， with certain characteristics. The diagnosis correct rate of yolk sac tumor and chocolate cyst with ultrasound was 86.5%. Conclusions： The ultrasound image of ovarian yolk sac tumor and chocolate cyst is of certain characteristics. Knowledge of the ultrasound images characteristics of yolk sac tumor and chocolate cyst helps improving the diagnostic yield corresponding when combining with the patient’s age， serum alpha-fetoprotein levels， which is of great value for clinical diagnosis.

卵巢的卵黄囊瘤又被称为内胚窦瘤，是临床上一种比较少见的主要发生于儿童以及年轻妇女的生殖系统恶性肿瘤，预后比较差，一般情况下3年的存活率仅仅有13%[1]。尽管对于卵黄囊瘤的病理以及临床特征已经有比较全面的报道了[2]，但是对于其影像学方面的描述还是很少的，同时也是不全面的。而对于巧克力囊肿，是女性患者的子宫内膜异位症的最常见形式，一般发病年龄多为30～40岁[3]，并且近年来其发病也有着明显的上升趋势，同时此病还有可观的家族倾向性[4]。本研究旨在通过探讨卵巢的卵黄囊瘤伴发巧克力囊肿的超声图像特征，同时与手术病理检查结果作对比，以分析出超声图像对于其的临床诊断价值。现将具体报道如下。 (郝美嘉 范静 柯伟)
