【摘要】目的：探讨退变小关节形态变化与退行性腰椎滑脱(DS)发生原因。方法：随机抽取腰椎CT扫描，L4/5椎小关节退变无与伴椎体滑脱各40例图像资料，对退变小关节损害程度进行分级，对两组病例关节突间距及关节面冠状角进行测量对比分析。结果：两组病例关节突间距和关节面冠状角有显著的差异性(P<0.01)，退变小关节的损害呈矢向性逐渐加重，DS组关节短缩更明显，关节面更接近矢状。结论：退变小关节损害是一矢向性逐渐加重过程，关节突短缩、关节面更接近矢状是DS发生过程中小关节形态与结构的变化，造成原因可能与下腰部脊椎前倾力和剪切力负荷随退变和失稳程度的增加而再加大有关。
