胎儿单脐动脉的产前咨询与临床分析(1)
【摘 要】 目的 探討胎儿单脐动脉的临床特点及妊娠结局咨询，指导单脐动脉的妊娠护理。方法 选择2016年3月至2018年3月68例单脐动脉妊娠妇女。回顾性分析孕妇的临床特点及妊娠结局。本文对单脐动脉合并胎儿畸形及染色体异常的情况进行了研究。结果 68例孕妇中单胎妊娠63例，双胎妊娠5例。结论 单纯单脐动脉孕妇来说，大多实现了良好的预防，在进行产前咨询之后，建议孕妇继续妊娠，同时保持定期的随访，而对于单脐动脉合并多发畸形孕妇来说，如果出现染色体异常的情况，建议孕妇终止妊娠，通过进行正确的产前咨询，能够为孕妇的孕期保健提供可靠的保障。

Abstract Objective To explore the clinical characteristics and pregnancy outcome consultation of fetal single umbilical artery， and to guide pregnancy nursing of single umbilical artery. Methods 68 pregnant women with single umbilical artery from March 2016 to March 2018 were selected. The clinical characteristics and pregnancy outcomes of pregnant women were retrospectively analyzed. In this paper， the single umbilical artery with fetal malformation and chromosomal abnormality were studied. Results Among the 68 pregnant women， there were 63 singleton pregnancies and 5 twin pregnancies. Conclusion Most pregnant women with single umbilical artery have achieved good prevention. After prenatal consultation， it is suggested that pregnant women continue to be pregnant and keep regular follow-up. For pregnant women with single umbilical arteries with multiple abnormalities， if chromosomal abnormalities occur， the pregnant women are proposed to terminate their pregnancy and be correct. Prenatal consultation can provide reliable protection for pregnant women during pregnancy.

胎儿和母亲的物质交换主要通过脐带进行，由两个脐动脉和脐静脉组成，它们周围都有着华通氏胶包裹，从而实现了保护血管的作用，同时能够更好的起到弹性缓冲，避免孕妇的血管受到直接的压力[1]。如果在脐带中只有1条动脉，则为单脐动脉。在孕妇的妊娠期，通过产前咨询检查，能够及时的发现单脐动脉，进一步检查明确是否合并其他的畸形，以及畸形的严重程度等情况进行分析，对孕妇孕期进行正确的产前指导，避免出现新生儿缺陷的情况。在对胎儿的单脐动脉进行诊断之后，产前检查咨询能够更好的指导孕期保健，为优生优育提供可靠的保障。因此，本研究主要通过对本院在2016年3月到2018年3月诊治68例单脐动脉孕妇资料进行回顾性分析，总结胎儿单脐动脉的产前咨询，对胎儿出现畸形或染色体异常进行分析。
1 资料与方法
1.1 孕妇资料 选取本院在2016年3月到2018年3月诊治的68例单脐动脉孕妇资料进行回顾性分析，对孕妇的产前检查，了解胎儿的畸形以及染色体异常情况。在此次选取的样本孕妇当中，年龄22～34岁，平均(26.32±2.57)岁。在此次选取的样本孕妇当中，排除了具有其他严重疾病的情况。所有样本孕妇均了解了此次研究的目的，在自愿的基础上参与到了此次研究当中。
1.2 方法
1.2.1 超声检测
孕妇超声检查使用的仪器是VuluSoN8和VuluSo730专家彩色超声诊断仪，使用二维凸探头，频率为3.5MHz。孕妇采用仰卧位进行胎儿全方位检查、多切面的检查，观察胎儿生长指数(胎儿双顶径、股骨长度、头围、羊水指数、腹围以及胎盘成熟度等)。在彩色血流叠加后，对胎儿两侧进行扫描分析胎儿腹腔段脐动脉，观察脐动脉和脐血流的各种信号特征。此外对胎儿的心脏、四肢、颅脑以及肾脏等畸形发生部位进行动态观察，对胎儿的畸形情况进行判断[2]。, 百拇医药(王力 胡君玉)
