血清C肽、糖化血红蛋白检验在糖尿病诊断中的临床价值(1)

     [摘要] 目的 探讨血清C肽、糖化血红蛋白检验在糖尿病诊断中的临床价值,为糖尿病检测提供科学可靠地依据。方法 以该院2012年3月—2015年6月收治的160例糖尿病患者为研究对象,根据糖尿病类型分为糖尿病组(98例患有单纯2型糖尿病患者)和糖尿病肾病组(62例患有糖尿病肾病患者),另外选取同期健康体检者45名作为对照组。检测各组患者血清C肽、HbA1c、空腹血糖以及用餐后2 h血糖等指标变化。 结果 糖尿病组和糖尿病肾病组患者血清C肽水平显著低于对照组,血清HbA1c、FBG、餐后2 h血糖明显高于对照组(P<0.05);糖尿病肾病组患者的血清C肽水平显著低于对照组患者,血清HbA1c含量明显高于对照组(P<0.05);糖尿病组和糖尿病肾病组患者的FBG和餐后2 h血糖含量差异无统计学意义(P>0.05);血清HbA1c含量与血清C肽呈负相关和FBG呈正相关关系(P<0.05)。结论 糖尿病发生时患者血清C肽和HbA1c水平变化明显,可以作为诊断糖尿病,判定病情严重程度的重要参考指标。

    Clinical Value of Serum C Peptide and Glycosylated Hemoglobin Test in Diagnosis of Diabetes Mellitus

    [Abstract] Objective To investigate the clinical value of serum C peptide and glycosylated hemoglobin test in the diagnosis of diabetes, and provide a scientific basis for the diagnosis of diabetes. Methods 160 patients for Marth 2012 to June 2015 with diabetes were divided into diabetic group (98 patients with type 2 diabetes) and diabetic nephropathy group (62 patients with diabetic nephropathy), and 45 cases of healthy physical examination were selected as control group, Serum C peptide, HbA1c, fasting blood glucose and 2H blood glucose were detected. Results Serum C level was significantly lower in the diabetic group and diabetic nephropathy group than in the control group, serum FBG, C and postprandial 2 h blood glucose were significantly higher than those in the control group (P<0.05). Serum HbA1c level was significantly higher than that in control group (P<0.05), FBG and 2h in diabetic group and diabetic nephropathy group were not significantly different (P>0.05); serum C level was negatively correlated with serum FBG peptide (P<0.05). Conclusion Serum levels of C and HbA1c in patients with diabetes mellitus can be used as an important reference index for the diagnosis of diabetes mellitus.

    糖尿病(diabetes mellitus,DM)是一种由于机体胰岛素分泌缺陷或者是胰岛素作用障碍导致的以血糖升高为主要特征的代谢性疾病[1]。目前本病主要依据1999年联合国世界卫生组织(World Health Organization ,WHO)修订的糖尿病诊断标准进行诊断。主要包括有临床症状(多饮、多尿、多食、体重减少等),空腹血糖(fasting blood-glucose,FBG)和葡萄糖负荷后2 h血糖含量,但是此法受暂时性血糖浓度影响大,且不能反映疾病的严重程度[2]。近年来研究发现糖化血红蛋白(hemoglobin A1c,HbA1c)可作为诊断糖尿病,判定病情严重程度的一项重要指标[3]。血清C肽可以反映机体胰岛B细胞功能,用于糖尿病分型[4]。该院通过检测血清C肽和HbA1c诊断糖尿病取得良好效果,现报道如下。, 百拇医药(付晓杰 王琳)
