[摘要] 目的：研究经翼点入路切除颅咽管瘤的常用手术间隙的应用和下丘脑重要血管的保护方法。方法：22例经翼点入路显微手术切除颅咽管瘤。结果：18例患者达到肿瘤全切除，3例达到次全切除,1例大部切除。术后16例患者出现多饮多尿，15例患者有血钠紊乱。术后随访1～4年，18例全切除的患者中有1例复发，行二次手术治愈。结论：充分利用适当的手术间隙，保护好下丘脑的结构和穿通动脉，可以降低下丘脑损伤的发生率并提高肿瘤的切除率，获得良好的疗效。
