促红细胞生成素在急性肾功能衰竭患者中的临床应用观察(1)

     [摘要] 目的:了解促红细胞生成素治疗对于急性肾功能衰竭患者肾衰竭临床指标及预后的影响。方法:急性肾功能衰竭患者共42例,分为两组,治疗组在基础治疗的基础上,给予促红细胞生成素5 000 U,每周2次,对比两组患者少尿期持续时间、肌酐减半时间、血红蛋白、红细胞压积、白蛋白等指标。结果:促红细胞生成素治疗组比对照组患者少尿期有所缩短(P<0.05),肌酐减半时间缩短(P<0.05),血红蛋白及红细胞压积水平较治疗前上升(P<0.01,P<0.05),两组患者治疗后血压变化无显著性差异,所有患者未出现血管栓塞性疾病、难以控制的高血压等并发症。结论:促红细胞生成素可应用于急性肾功能衰竭患者的治疗,可以改善其预后,且短时间使用其严重不良反应罕见。

    Observation of the clinical effect of the use of erythropoietin in acute renal failure

    [Abstract] Objective: To evaluate the impact of erythropoietin (EPO) on clinical parameters of patients with acute renal failure (ARF) and the prognosis. Methods: A total of 42 eligible patients with ARF were randomly divided into two groups. Besides routine treatments for ARF, patients in therapeutic group were administrated with 5 000 U EPO, twice a week. Clinical parameters of patients in therapeutic group and control group such as duration of oliguria stage, time for creatinine decreased in half, hemoglobin, hematocrit, and albumin and so on were compared. Results: Duration of oligura stage in therapeutic group was significantly reduced compared with control group (P<0.05), and time for creatinine decreased in half was also significantly shortened (P<0.05). In the two groups, hemoglobin and hematocrit after treatments were higher than those before treatments (P<0.01, P<0.05), but there was no difference in blood pressure between the two groups after treatments. Severe side effects such as vascular thromboenbolism disease and refractory hypertension were not detected in any patients. Conclusion: Erythropoietin could be used in ARF because it may improve patients' prognosis and no severe side effect was observed during its short-term administration.
