[摘要] 目的：观察不同急救方案对急性心肌梗死(AMI)患者的治疗效果。方法：选择2009年1月～2011年1月我科收治的120例AMI患者为研究对象，分为对照组、尿激酶组(UK组)和重组组织型纤溶酶原激活剂组(rt-PA组)，每组各40例。对照组给予镇静、止痛等一般处理，皮下注射低分子肝素钙，口服肠溶阿司匹林等常规治疗。UK组在对照组基础上给予UK。rt-PA组在对照组基础上给予rt-PA。比较组间患者治疗效果和价效比。结果：rt-PA组心绞痛发生率(7.5%)明显低于UK组(25.0%)(P<0.05)；rt-PA组及UK组心绞痛、再发心肌梗死、心脏衰竭、住院病死发生率均明显低于对照组(P<0.05或P<0.01)。rt-PA组的价效比(15 309.3)高于UK组(2 976.8)(P<0.01)。结论：rt-PA溶栓效果好于UK，是不具备介入治疗条件时的首选再灌注方案。

Comparison on different emergency medical plans for curing acute myocardial infarction patients

[Abstract] Objective: To observe the curative effect of different emergency medical plans for AMI patients. Methods: 120 AMI patients in our department from January 2009 to January 2011 were divided into control group, UK group and rt-PA group, with 40 patients in each group. Patients in control group were got ordinary treatment as sedation, pain-killer conventional therapy, and Hypodermic Injection of low molecular heparin calcium and taking orally Aspirin Enteric Microencapsulated Capsules. UK group was added Uk treatment on base of control group. rt-PA group was added rt-PA on base of contrast group. Then took comparison on the curative effects and cost-effectiveness ratios in these groups. Results: The angina incidence rate in rt-PA (7.5%) group was obviously lower than that in UK group (25%) (P<0.05); The incidence rate of angina, recurrent myocardial infarction, heart failure and ill death in hospiatal in rt-PA group and UK group were obviously lower than that of the control group (P<0.05 or P<0.01). In the point of cost-effectiveness ratio, rt-PA group (15 309.3) was higher than UK group(2 976.8) (P<0.01). Conclusion: Thrombolysis effect of rt-PA is better than UK, and it is the first reperfusion plan when treatment conditions are not available.
