母体乙肝病毒携带与新生儿黄疸发生相关性关系的研究(1)

[摘要] 目的 探讨母体乙肝病毒携带与新生儿黄疸发生的关系。 方法 回顾性分析我院2007年5月~2010年8月足月产新生儿病理性黄疸患儿220例及正常对照组新生儿100例的临床资料,分别统计新生儿病理性黄疸组与对照组新生儿性别、体重、G6PD缺乏、总胆红素水平、患儿与母体ABO或Rh血型不合以及母体年龄、吸烟、乙肝病毒感染情况。 结果 Logistic回归分析显示血型系统不合、G6PD水平、性别因素是新生儿病理性黄疸发生的独立风险因素,而母体是否感染乙肝病毒则不是发病的独立风险因素。患儿母体乙型肝炎抗原阳性组其血清总胆红素水平与HBsAg乙型肝炎抗原阴性组相比较,差异有高度统计学意义(P < 0.01)。其中母体乙型肝炎抗原阳性组G6PD缺乏症型黄疸发生率显著高于(P < 0.01),而母体乙型肝炎抗原阳性患儿组新生儿溶血病及其他类型黄疸与乙型肝炎抗原阴性组比较,差异无统计学意义(P > 0.05)。 结论 母体乙肝病毒感染不是新生儿黄疸发病的独立危险因素,却加大G6PD缺乏患儿患新生儿黄疸的危险,并与G6PD缺乏症病情有关。

Study on relationship of hepatitis B virus in mother and neonatal jaundice

[Abstract] Objective To investigate the clinical relationship of hepatitis B virus in mother and the development of neonatal jaundice. Methods 220 cases of newborn infants with neonatal jaundice and 100 cases of normal newborn infants who born in our hospital from May 2007 to August 2010 were retrospectively analyzed. The newborn infants' gender, body weight, G6PD deficiency status, total bilirubin levels, mother and newborn blood type consistency status, mother age, smoking and hepatitis B virus infectious status were also analyzed. Results Logistic regression analysis indicated that mother and newborn blood type consistency status, G6PD deficiency status and neonatal gender were independent risk for the development of neonatal jaundice, however, mother hepatitis B virus infectious status was not. There was a significant difference on the serum total bilirubin level between children with mother of hepatitis B virus positive group and hepatitis B virus negative group(P < 0.01).There was significant difference in the incidence of neonatal G6PD between hepatitis B virus positive group and hepatitis B virus negative group (P < 0.01). However, there was no significant difference in neotatal hemolytic disease and other type neonatal jaundice between the hepatitis B virus positive group and hepatitis B virus negative group (P > 0.05). Conclusion Mother hepatitis B virus infectious status is not independent risk for the development of neonatal jaundice, however, which can increase the risk for the development of G6PD disease ......
